血清半胱氨酸蛋白酶抑制剂C在狼疮性肾炎中的意义

【摘要】 目的探讨血清半胱氨酸蛋白酶抑制剂C(Cysc)检测在狼疮性肾炎早期诊断中的意义。方法对56例系统性红斑狼疮患者和50例正常对照组血清进行血清肌酐(SCr)和血清半胱氨酸蛋白酶抑制剂C(CysC)测定,同时进行尿蛋白定性测定,并作相关分析。SCr采用肌氨酸氧化酶法,CysC采用免疫比浊法,尿蛋白定性试验采用干化学分析法。结果正常对照组和SLE患者SCr分别为(72.80±14.70)和(226.85±45.80),Cysc分别为(0.95±0.21)和(3.88±1.05);两组比较,SLE患者SCr和Cysc较对照组明显增高(P<0.01),SLE患者尿蛋白定性阳性组较尿蛋白定性阴性组血清SCr、Cysc有明显增高(P<0.01),SLE患者血清Cysc阳性率较SCr明显增高(P<0.01)。结论血清Cysc是反映狼疮性肾炎的一个敏感性指标。

【Abstract】 【Objective】To investigate the significance of serum cystatin C in the early diagnosis of lupus nephritis.【Methods】Serum creatinine(SCr) and serum cystatin C(SysC) of 56 SLE patients and 50 normal controls were detected by Hitachi 7600 automatic biochemical analyzer.At the same time,qualitative test of urinary protein and relative analysis were done.Serum creatinine(SCr) was detected with sarcosine oxidase method,serum cystatin C(SysC) with immunoturbidimetry assay,and qualitative test of urinary protein with dry-chemistry method.【Results】SCr of normal control group and SLE patients were respectively(72.80±14.70) and(226.85±45.80);Cysc were(0.95±0.21) and(3.88±1.05).By contrast,the expression of serum cystatin C and serum creatinine in systemic lupus erythematosus patients was significantly higher than that in the control group(P <0.05).The expression of serum cystatin C and serum creatinine in the positive group of urinary protein was significantly higher than that of the negative group in systemic lupus erythematosus patients(P <0.05).The positive expression rate of serum cystatin C was significantly higher than that of serum creatinine in systemic lupus erythematosus patients(P <0.05).【Conclusion】 Serum cystatin C is a sensitivity index in lupus nephritis.
