尿路感染的病原菌分布特征及耐药性检测分析
Distribution characteristics of pathogens and antibiotic resistance testing analysis in urinary tract infection
【摘要】 目的了解川北地区尿路感染的病原菌分布及对抗菌药物的耐药情况。方法分离自川北医学院附属医院泌尿系统感染患者尿液标本的细菌,采用法国生物梅里埃(API)公司生产的Bio-Merieux VITEK-32全自动细菌鉴定系统对细菌进行鉴定,用琼脂稀释法进行药物敏感试验,双纸片表型确证试验检测产ESBLs菌株,结果用美国临床实验室标准化协会(CSLI)2006公布的标准判定。结果检出的442株尿路感染的细菌中,革兰阴性菌占71.9%,以大肠埃希菌为主,占42.1%。大肠埃希菌除对亚胺培南全部敏感外,对其它抗菌药均表现出不同程度耐药。186株大肠埃希菌中,产ESBLs 85株,检出率为45.8%。结论川北地区尿路感染的主要致病菌为肠杆菌科细菌,不同病原对抗菌药物的敏感性不同。尿路感染的治疗中,医师应根据病原鉴定和药敏试验结果选择恰当的抗菌药。
【Abstract】 Objective To study distribution characteristics of pathogens and the resistance to antimicrobial drugs in urinary tract infection in North Sichuan Area.Methods The bacillus of isolated from the urine specimens of Patients with urinary tract infections in Affiliated Hospital of North Sichuan Medical College were evaluated by the system of Bio-Merieux VITEK-32.The agar dilution method was used for susceptibility test of 21 antimicrobial agents and ESBLs were detected by double paper phenotype corroborate test.The results were ascertained according to the Clinical and Laborarory Standarads Institute(CSLI).Results In 442 stains of urinary tract infection,Gram-negative bacteria accounted for 71.9% and Escherichia coli was the most common becteria (42.1%).All of 186 stains of Escherichia coli were sensitive to imipenem,but to other 20 antimicrobial agents show the different degree resistant rates.ESBLs had detected in 85 stains of Escherichia coli,the detection rates for 45.8%.Conclusion The most common becteria in urinary tract infection was Escherichia coli and different pathogens had different sensitivity to antimicrobial agents in North Sichuan Area.In treatment of urinary tract infection,clinicans should be appropriate selected antimicrobial agents basing on identifying pathogen and antimicrobial susceptibility test results.
