雷帕霉素药物涂层支架CYPHER plus+治疗冠脉前降支长病变临床应用

【摘要】 目的评价雷帕霉素药物涂层支架(CYPHER plus+,Cordis)治疗冠状动脉前降支长病变的长期临床疗效。方法前降支长病变(病变长度≥20mm)患者46例置入CYPHERplus+支架56个,病变长度20~45mm,平均(25.2±7.8)mm,术前病变狭窄程度(87.9±7.5)%;血管直径(3.0±0.75)mm。其中前降支病变>28mm10例,置入2个支架重叠,1个长支架覆盖病变36例。结果手术即刻成功率100%,术后造影病变残余狭窄(3.5±2.8)%,支架覆盖病变完全,支架近、远端无夹层,血流TIMI3级。随访6个月,心绞痛消失43例,术后6个月复查冠状动脉造影,无血管再狭窄发生。结论雷帕霉素药物涂层支架治疗前降支长病变是解决前降支长病变安全有效的方法 。

【Abstract】 Objective To evaluate the clinical effect of CYPHER plus + Cordis( Rapamycin) plus stent in treatment of left anterior decending( LAD) long leision. Methods LAD long leision. ( the length of lension≥20 mm) ,46 patients with coronary heart disease( CAD) were treated with( CYPHER plus + ,Cordis) stent and 56 stent were planted in 46 case LAD leision. the length of lension was from 20 mm to 45 mm,( 25. 2 ± 7. 8) mm,the degree of stenosis before procedure was( 87. 9 ± 7. 5) % ,and the vessel diameter before procedure was( 3. 0 ± 0. 75) mm. 10 lesions over 28 mm long were teated with two overlapped stents to cover the whole lesion,36 leision were teated with only one stent. Results Immedeiate procedure success rate was 100% ,with residual stenosis of( 3. 5 ± 2. 8) % ,All the lesions were covered fully by stents and the coronary flow reached TIMI grade three. No new dissection was observed in the optimal of stents. During the 6 months follow-up,the symptom of angina was disappeared in 43 patients. all patients received CAG in 6 months and no restenosis was observed. Conclusion Duge eluting the stents( CYPHER plus + ,Cordis) implantation is a safe and efficient therapy in treating patients with LAD long leision.
