坎地沙坦对老年慢性心力衰竭伴阵发性房颤患者P波最大时限及P波离散度的影响

【摘要】 目的探讨坎地沙坦对慢性心力衰竭伴阵发房颤患者P波最大时限(Pmax)及P波离散度(Pd)的影响。方法 51例慢性心力衰竭伴阵发性房颤患者被随机分为坎地沙坦治疗组(治疗组26例)和对照组(25例),慢性心力衰竭基本治疗两组相同,治疗组加用坎地沙坦2~4mg/d,在治疗前、治疗后6个月分别测量Pmax和Pmin,计算出Pd(Pd=Pmax-Pmin)。结果治疗前两组Pmax和Pd比较无显著差异(P>0.05);治疗6个月治疗组的Pmax、Pd显著降低,与对照组相比有显著意义(P<0.01);而对照组治疗前后相比没有差异(P>0.05)。治疗组房颤发作次数明显减少,与对照组比较差异有显著意义(P<0.01),结论坎地沙坦治疗慢性心力衰竭伴阵发性房颤患者可明显降低Pmax、Pd,减少房颤发作次数。

【Abstract】 Objective To investigate the effects of candesartan on Pmax and P wave dispersion( Pd) in chronic cardiac insufficiency patients with paroxysmal atrial fibrillation( PAF). Methods 51 chronic cardiac insufficiency patients were randomly divided into candesartan treatment group( treatment group。n = 26) and control group( n = 25). Basic treatment of chronic cardiac insufficiency in two groups was comparable. The pa-tients in treatment group were treated by candesartan 2 ~ 4 mg/d. Pmax and Pd were measured based on the 1 2-lead electrocardiogram at the beginning and at the six months after taking medicine. Results At the beginning there was no significant difference between two groups( P > 0. 05) ; Pmax and Pd were significantly decreased in treatment group after 6 months compared to that of control group ( P < 0. 01) ; but it was not significant in control group ( P > 0. 05). PAF rate was decreased after 6 months treatment and compared to that of control group ( P <0. 01). The difference was significant in control groups( P < 0. 01). Conclusion Pmax and Pd were obviously decreased in chronic cardiac insufficiency patients with PAF. Candesartan can decrease PAF rate by decreasing Pmax and Pd.
