【摘要】 目的探讨三维适形放疗非小细胞肺癌近期疗效及其影响因素。方法三维适形放疗110例未手术非小细胞肺癌患者,于放疗前行99mTc-MIBI肺显像。应用χ2和Logistic多元回归分析方法研究肺癌患者临床病理资料、物理参数及99mTc-MIBI肺显像指标与三维适形放疗近期疗效的关系。结果放疗后完全缓解29.0%(32/110),部分缓解58.2%(64/110),全组患者有效率87.3%(96/110)。单因素分析有统计学意义的相关因素是卡氏评分、血红蛋白水平、肿瘤大小、淋巴结转移情况、放疗时间、放疗增敏情况及ER和DR(P<0.05);经Logistic回归模型分析,血红蛋白、肿瘤大小、放疗时间、放疗增敏及DR为影响3D-CRT近期疗效的独立因素(P<0.05)。结论3D-CRT能有效缓解患者的症状,血红蛋白、肿瘤大小、放疗时间、放疗增敏及DR为影响3D-CRT近期疗效的独立因素。更多还原

【Abstract】 Objective To study the influential factors of three-dimensional conformal radiotherapy in the treatment of non-small cell lung cancer patients. Methods 110 non-small cell lung cancer (NSCLC) patients were treated by three-dimensional conformal radiotherapy. The clinical pathological and physical indexes of 99mTc-MIBI parameters were analyzed using Chi-Square test and Logistic regression model. Results Complete response and partial response were achieved in 32 and 64 patients respectively. The overall response rate was 87.3%. The univariate analysis revealed that many parameters(Grade of Karnofsky,level of Hb,tumor size,Lymph node metastasis,time of radiotherapy,radiosensitization,ER,PR)were significantly associated with the recent effect of three-dimensional conformal radiation therapy (P<0.05). Logistic analysis revealed that level of Hb,tumor size,time of radiotherapy,DR were associated with the recent effect of three-dimensional conformal radiation therapy (P<0.05). Conclusion Three-dimensional conformal radiotherapy is effective for NSCLC patients in terms of symptom relief. Levels of Hb,tumor size,time of radiotherapy,radiosensitization,DR are associated with the recent effect of three-dimensional conformal radiation therapy.更多还原
