纳米炭混悬液在术中探寻非小细胞肺癌前哨淋巴结的应用

【摘要】 目的探索应用纳米炭混悬液术中探寻非小细胞肺癌(non-small cell lung cancer,NSCLC)前哨淋巴结(sentinel lymph node,SLN)的识别率及其判断区域淋巴结转移情况的准确性。方法15例ⅠA至ⅡB患者,术中瘤周注射纳米炭混悬液,并行肺叶切除术与系统性淋巴结廓清术。染色淋巴结定义为SLN。结果共11例患者成功识别SLN(识别率73.3%),SLN区域淋巴结判断的敏感率、准确率、假阴性率分别为85.7%、90.9%和14.3%。其中1例cTNMⅠA患者发现2枚跳跃性转移SLN。结论初步证实应用纳米炭混悬液探寻NSCLC前哨淋巴结是一种实用的判断区域淋巴结状态的方法。

【Abstract】 Objective To determine sentinel lymph node(SLN) identification rate(IR) using carbon nanoparticles suspension injection in patients with NSCLC and to evaluate the accuracy of SLN for the presence of regional metastasis.Methods 15 patients with clinical stage Ⅰ A to Ⅱ B,who finally underwent lung resection and systematic nodal dissection for NSCLC,were prospectively analyzed.Carbon nanoparticles suspension injection was injected in the peritumoral tissue and the lymph nodes to stain was identified as a sentinel node.Results SLN was identified in 11 patients(IR = 73.3%).Sensitivity,accuracy and false negative rate of the sentinel lymph node in predicting the status of lymph node stations were respectively 85.7%,90.9%,and14.3%.2 skip metastasis SLN were found in 1 cTNM I A patient.Conclusions Using carbon nanoparticles suspension injection of SLN biopsy is a useful clinical procedure in predicting the status of lymph node stations in patients with NSCLC.
