肺炎克雷伯菌多药耐药株的耐药机制研究

【摘要】 目的调查1株多药耐药肺炎克雷伯菌(MDRKP)58种耐药相关基因与可移动遗传元件的存在情况。方法对分离自我国内地的1株MDRKP,采用聚合酶链反应(PCR)及序列分析的方法分析58种β-内酰胺类、氨基糖苷类、喹诺酮类和其他广谱抗菌药物耐药相关基因与可移动遗传元件。结果该株MDRKP检出TEM-1、SHV-11(均测序比对证实)t、etA、mdfAt、rbCt、np513I、SEcp1基因,且gyrA基因存在突变,其余50种基因未检出。结论该菌株耐Ⅰ~Ⅲ代头孢菌素抗菌药物,可能与同时产TEM-1型和SHV-11型β-内酰胺酶引起牵制耐药机制有关,该株MDRKP表型为多药耐药与gyrA基因突变及获得可移动遗传元件和多种耐药基因相关,在肺炎克雷伯菌中检出ISEcp1为国内首次报道。

【Abstract】 OBJECTIVE To investigate the distribution of 58 kinds of resistance genes and mobile genetic elements in a multi drug resistant Klebsiella pneumoniae(MDRKP) strain.METHODS This MDRKP strain was collected from a hospital in China.Then,58 kinds of genes resistant to beta-lactams,aminoglycosides,quinolones,and other broad-spectrum antimicrobial agents as well as mobile genetic elements were analyzed by PCR and verificated by DNA sequencing.RESULTS TEM-1,SHV-11(both were verificated by DNA sequencing),tetA,mdfA,trbC,tnp513,ISEcp1 genes and a gyrA mutant were detected in this MDRKP strain.However,the rest 50 kinds of genes couldn′t be detected.CONCLUSION The MDRKP strain resistant to the first to third generation Cephalosporins might be relative with trapping mechanism caused by producing overmuch TEM-1 and SHV-11 genes synchronously.Furthermore,multi drug resistance phenotype of this MDRKP strain might be relative with a gyrA mutant,and 7 kinds of resistance genes and mobile genetic elements obtained.Moreover,it′s the first report that ISEcp1 is detected in K.pneumoniae in China.
