呼吸机相关性肺炎病原菌的耐药性研究

【摘要】 目的探讨呼吸机相关性肺炎(VAP)感染病原菌的耐药性,以制定预防与控制方案。方法对211例VAP患者进行了感染病原菌种类、耐药率的回顾性调查与分析。结果 211例VAP患者均患有严重的基础疾病,分别进行过气管插管、气管切开或呼吸机治疗,使用过激素和广谱抗菌药物;VAP感染病原菌主要为鲍氏不动杆菌、铜绿假单胞菌、肺炎克雷伯菌、嗜麦芽寡养单胞菌、大肠埃希菌等,以上病原菌均为多药耐药菌;其中,泛耐药株检出率达到8.1%。结论 VAP病原菌耐药性已非常严重,应及时进行标本细菌培养,根据病原学检测结果合理使用抗菌药物。

【Abstract】 OBJECTIVE To approach the antimicrobial resistance of pathogens in ventilator-associated pneumonia(VAP) patients to institute the plans of prevention and controll.METHODS The species of pathogens and the rate of antimicrobial resistance in the 211 VAP patients were reviewed retrospectively and analyzed statistically.RESULTS The 211 patients with VAP had all severe underlying diseases and taken the treatment of tracheal intubation,incision of trachea or ventilator,and prescription of hormone and broad-spectrum antibiotics.The main pathogens were Acinetobacter baumannii,Pseudomonas aeruginosa,Klebsiella pneumoniae,Stenotrophomonas maltophilia,Escherichia coli,etc,which all possessed multidrug resistance.The detection rate of pan-resistant strains was 8.1%.CONCLUSIONS The antimicrobial resistance of pathogens in VAP patients is very serious,the specimen bacterial culture should be performed in time,and the clinician should prescribe antibiotics reasonably according to etiological detection.
