【摘要】 目的探讨多通道微创经皮肾镜结合钬激光治疗复杂性肾结石的临床应用价值。方法自2003年3月至2009年5月对657例复杂性肾结石患者进行随机分组,其中多通道微创经皮肾镜治疗组367例,单通道经皮肾镜治疗组290例,比较两组的手术时间,术中出血量,住院时间,治疗总费用及术后感染等并发症发生率。结果与单通道经皮肾镜碎石组相比,多通道微创经皮肾镜组手术时间无明显差异(130.35min∶132.02min,P>0.05),术中出血少(30.022±2.89ml∶50.25±5.9ml,P<0.05),术后住院时间短(10.6±1.09d∶15.5±1.23d,t=5.36,P<0.05),治疗总费用低(10255.50±36.88元∶15098.35±99.67元,P<0.05),术后感染发生率明显小(9%∶29%,P<0.05)。结论多通道微创经皮肾镜结合钬激光治疗复杂性肾结石具有出血少,住院费用低,并发症发生率低等优点。更多还原

【Abstract】 Objective To explore the techniques and efficacy of multiple tracts minimally invasive percutaneous nephrolithotomy with holmium laser for the treatment of complex kidney stones. Methods From March 2003 to May 2009, 657 patients with complex kidney stone were divided to two group randomly. 367 patients (group 1) underwent multiple tracts minimally invasive percutaneous nephrolithotomy, 290 cases (group 2) underwent single tract percutaneous nephrolithotomy. The mean operative time, blood loss, hospital stay, total cost and postoperative complications such as the incidence of infection were compared. Results There were no significant difference in operative time (130.35 min VS 132.02 min, P> 0.05), blood loss 30.022 ± 2.89 ml and 50.25 ± 5.9 ml (P< 0.05), postoperative hospital stay 10.6 ± 1.09 d and 15.5 ± 1.23 d(t=5.36, P<0.05), total cost of treatment 10255.50 ± 36.88 yuan and 15098.35 ± 99.67 yuan (P< 0.05), postoperative infection 9% and 29% between the two groups(P<0.05). Conclusions Multiple tracts minimally invasive percutaneous nephrolithotomy with holmium laser for the treatment of complex kidney stones has the advantages of shorter hospital stayt, low cost, low incidence of complications and it’s worthy to be extended.更多还原
