【摘要】 目的探讨survivin和caspase-3在食管鳞癌术前放疗前后的表达及其与预后的关系,为食管癌的个体化治疗提供理论依据。方法对56例中晚期食管鳞癌患者采用术前放疗加手术,用PV600通用型二步法免疫组化检测survivin和caspase-3于术前放疗前后的表达水平,分析二者放疗前后表达的差异及其与预后的关系。结果放疗前活检标本中survivin蛋白表达为0.43±0.24,而放疗后手术标本的survivin蛋白表达0.33±0.27,t=2.735,P=0.08,无统计学意义;放疗前活检标本中caspase-3蛋白表达为0.33±0.18,而放疗后手术标本的caspase-3蛋白表达为0.43±0.27,t=-2.986,P=0.04,有统计学意义。1,2,3,5年生存率分别为87.5%、67.5%、59.5%和29.9%。放疗前后survivin和caspase-3蛋白表达与生存期均无明显相关性。结论食管癌的预后因素是非常复杂的,caspase-3和Survivin与生存期的关系还需进一步探索。更多还原

【Abstract】 Objective To study the expression levels of survivin and caspase-3 in esophageal squamous cell carcinoma before and after radiotherapy and their relation to prognosis,in order to provide theoretical basis for individualized treatment of esophageal carcinoma.Methods Fifty-six patients with middle-stage and advanced esophageal squamous carcinoma were treated with preoperative radiotherapy plus surgery.The expression levels of survivin and caspase-3 in biopsy samples from all the patients before and after radiotherapy were detected with the PV600 general type two-step immunohistochemical method.The difference in expression levels of survivin and caspase-3 before and after radiotherapy and its relation to prognosis of esophageal squamous cell carcinoma were analyzed.Results The expression levels of survivin in biopsy samples from all the patients before and after radiotherapy were 0.43±0.24 and 0.33±0.27,respectively(t=2.735,P=0.08,no statistical significance).The expression levels of caspase-3 in biopsy samples from all the patients before and after radiotherapy were 0.33±0.18 and 0.43±0.27,respectively(t=-2.986,P=0.04).The 1,2,3,5-year survival rates of the patients were 87.5%,67.5%,59.5% and 29.9%,respectively.The expression levels of survivin and caspase-3 in biopsy samples before and after radiotherapy were not related to prognosis.Conclusion Prognostic factors of esophageal carcinoma are very complex.The relationships of the expression levels of survivin and caspase-3 with the prognosis of esophageal squamous cell carcinoma need to be studied further.更多还原
