【摘要】 目的对CT引导下经皮肺穿刺冷循环射频消融治疗晚期肺癌的有效性和安全性进行研究评价。方法对32例102个不同病理类型的晚期肺癌行冷循环射频消融治疗,观察治疗过程中血压、脉搏和周围血氧饱和度的变化;治疗前后肝肾、肺功能的改变;以及CT扫描肿瘤组织中低密度影和肿瘤体积的变化。结果 32例晚期肺癌患者冷循环射频消融治疗中血压、脉搏及周围血氧饱和度无明显改变;治疗后肝肾功能、肺功能无明显改变;治疗后1个月内CT扫描见肿瘤体积较术前有所增大,3个月后102个肿瘤中出现CR 23个(100%缩小),PRa 53个(80%～100%缩小),PRb 24个(50%～80%缩小),NR 2个(缩小<50%);治疗后第3个月肿瘤内低密度影Ⅰ型2个,Ⅱ型43个,Ⅲ型57个。结论冷循环治疗晚期肺癌创伤小,近期疗效好,比较安全,可作为一种新的安全有效的治疗方法。更多还原

【Abstract】 Objective To study efficacy and safety of percutaneous cool-tip radiofrequency ablation guided by computerized tomography(CT) in the treatment of advanced lung cancer.Methods Thirty-two patients with advanced lung cancers of 102 different pathological types were treated with MERP technique.The patients′blood pressure,pulse rate and blood oxygen saturation were monitored during the procedure.The hepatic,pulmonary and renal functions were examined before and after the procedure.The tumor bulk and its low-density shadow(LDS) were compared with CT before and after the treatment.Results There was no any change in the blood pressure,pulse rate and blood oxygen saturation before,immediately after termination and one month after the procedure,nor did the hepatic,pulmonary and renal function.The bulk of the tumor enlarged by the end of the first month after the therapy.The CT scanning of the tumor at the end of the third month revealed that of 102 tumors,23 showed CR(100% regression),53 PRa(80%-100% regression),24 PRb(50%-80% regression),and 2 NR(less than 50% regression).By the third month,the LDS of 102 tumors detected by CT showed 2 in type Ⅰ(less than 50%),43 in type Ⅱ(50%-80%) and 57 in type Ⅲ(more than 80%).Conclusion The cool-tip radiofrequency ablation is a promising mini-invasive technique for the treatment of advanced lung cancer with satisfactory efficacy and safety.更多还原
