应用三叶钢板和骨诱导活性材料治疗老年性肱骨近端三、四部分骨折的近期疗效

【摘要】 目的:探讨应用三叶钢板固定术和填塞骨诱导活性材料治疗老年性肱骨近端三、四部分骨折的临床疗效。方法:回顾分析2000年1月~2006年7月收治的35例肱骨近端粉碎性骨折临床资料,其中男性21例,年龄61~80岁,平均71.5岁;女性14例,年龄60~77岁,平均65.5岁。按Neer分型,三部分骨折28例,四部分骨折7例。手术采用三角肌、胸大肌间隙入路,尽量保护周围软组织及肱骨头血供,修复肩袖和关节囊。结果:本组随访6~36个月,平均11.5个月,按Neer评分标准评分,优19例,良12例,可3例,差1例。优良率达88.6%。结论:对于老年性肱骨近端三、四部分骨折,运用三叶板固定和填塞骨诱导活性材料,固定牢固,利于早期功能锻炼,疗效满意。

【Abstract】 Objective:To evaluate the results of internal fixation with cloverleaf plate supplemented with active osteo-inductive material for three-part and four-part proximal humeral fractures in aged patients.Methods:Thirty-five cases of proximal humeral fractures in aged patients were treated with open reduction and cloverleaf plate fixation supplemented with active osteo-inductive material,among whom 28 were three-part proximal humeral fractures while 7 were four-part ones according to Neer classification.Reduction and fixation was done via deltopectoral-groove approach,preserving soft tissues and circulation,repairing rotator cuff and capsule.Results:All cases were followed up for 6-36 months,11.5 months on average.According to Neerg rading scale,the excellent and good rate was 88.6%.Conclusion:Internal fixation of three-part and four-part proximal humeral fractures of aged patients with cloverleaf plate combined with active osteo-inductive material is effective,with stable fixation,earlier rehabilitation,and satisfactory functional recovery.It appears to be a useful device to treat such case.
