2型糖尿病患者血清白介素23、干扰素γ水平与胰岛素抵抗相关性
The correlation of serum levels of IL-23 and IFN-γ with insulin resistance in type 2 diabetes
【摘要】 目的探讨2型糖尿病患者白介素23(IL-23)、干扰素γ(IFN-γ)与胰岛素抵抗(IR)相关性。方法研究对象分为正常对照组(NGT)20例、新诊2型糖尿病组(DM1)33例、既往2型糖尿病组(DM2)32例,各组根据BMI又分为正常体重亚组(NW,BMI≤25kg/m2)、肥胖亚组(OB,BMI>25kg/m2)。检测各组的体重、身高、腰围、臀围、FPG、FIns、TC、TG、HbA1c,酶免法测定IL-23、IFN-γ浓度,简易稳态模型评估胰岛素抵抗指数(HOMA IR)。结果 DM1、DM2组IL-23均较NGT组明显升高(P均<0.05),DM1组较DM2组明显升高(P<0.01),各组内NW和OB亚组间无统计学差异(P>0.05);NGT、DM1、DM2三组间比较IFN-γ无统计学差异(P>0.05)。血清IL-23与HOMA-IR显著相关(r=0.412,P=0.001),与IFN-γ无相关(P>0.05);IFN-γ与HOMA-IR显著相关(r=0.739,P=0.001)。结论 2型糖尿病患者IL-23显著升高,与HOMA-IR显著相关,IFN-γ无明显升高,提示IL-23是2型糖尿病IR的重要细胞因子,IL-23不通过参与调节IFN-γ发挥作用。
【Abstract】 Objective To investigate the serum levels of interleukin~23 (IL-23) and interferon-γ (IFN-γ) with insulin resistance in patient with type 2 diabetes mellitus.Methods Ninety type 2 diabetics were divided into three groups of the normal glucose tolerance (NGT),newly-diagnosed type 2 diabetes(DM1) and the previously diagnosed type 2 diabetes(DM2).All the patients were then subdivided into NGT,DM1 and DM2 groups with normal weight and obesity.The weight,height,waist and hip circumference,fasting plasma glucose(FPG),fasting insulin,total cholesterol,triglycerides and HbA1c as well as serum IL-23 and IFN-γ were determined.Insulin resistance was calculated by the homeostasis model assessment (HOMA-IR).Results Serum IL-23 levels in group DM1 and DM2 were increased compared with NGT group(P<0.05),the IL-23 level in DM1 group was increased compared with DM2 group(P<0.01),and there was no significant difference in IL-23 level between NW and OB subgroups of each group.There was no significant difference between any of DM1 and DM2 and NGT in IFN-γ level. IL-23 level correlated strongly with HOMA-IR(r=0.412,P=0.001),was not correlate with IFN-γ(P> 0.05).IFN-γ(r=0.739,P=0.001) was strongly associated with HOMA-IR.Conclusions Serum IL- 23 is significantly increased in type 2 diabetes mellitus,and strongly correlated with HOMA-IR.IFN-γ level shows no significant difference between group DM and NGT,indicating that IL-23 is an important mediator for insulin resistance in type 2 diabetes mellitus,producing a marked effect without adjusting IFN-γ.
