45例肺叶切除术后并发凝固性血胸的处理

【摘要】 目的:探讨肺叶切除术并发胸腔凝固性血胸的诊疗。方法:对2006年5月～2009年6月45例肺叶切除术并发胸腔凝固性血胸采用尿激酶胸腔内注入进行回顾性总结分析。结果:39例患者经用尿激酶胸腔内保留注射,胸内血块消失,余肺复张良好,有效率达86%,6例无效,需要再次开胸清除血块。结论:术后胸腔出血是肺叶切除手术的常见的并发症。往往通过胸腔闭式引流达到排出积血的目的,但是有部分病人因引流不畅,或是早期出血较快、较多形成血块。术后合理应用尿激酶胸腔内保留注入给药能起到很好的清除血块的作用。更多还原

【Abstract】 Objective:To investigate diagnosis and analyze treatments of pleural hemothorax coagulation (PHTC) after lobectomy surgery.Methods:To perform retrospective analysis based on the clinical data of 45 PHTC cases in our hospital from May 2006 to June 2009 who used intrapleural urokinase injection.Results:39 cases(out of 45 )showed that chest blood clots were disappeared,the efficiency ratio was 86%,but 6 cases were failed,which neededto re-operate for blood clots removal.Conclusion:Post-operative pleural hemorrhage is a common complication after lobectomy surgery.Normally it is treated by closed thoracic drainage for the purpose of clearance of blood clot.However to the patients who has blockage of the thoracic drainage or quite amount of blood clots caused by early hemorrhage,Application of intrapleural urokinase injection after surgery can play good effects of drug efficacy,should be considered as a suitable post-operative treatment of lobectomy.更多还原
