类风湿性关节炎患者外周动脉僵硬度评估

【摘要】 目的采用动脉硬化检测技术和超声回波跟踪(echo-tracking,ET)技术评估类风湿性关节炎(rheumatoid arthritis,RA)患者外周动脉僵硬度。方法连续纳入25例RA患者,同时选取48例健康自愿者作为对照组。采用全自动动脉硬化检测仪对所有受试者行肱-踝动脉脉搏波传导速度(branchial ankle pulse wave velocity,baPWV)检测,此指标是评估动脉僵硬度的经典指标之一。同时应用超声回波跟踪技术对所有受试者行动脉僵硬度及形态学检测。结果与对照组比较,RA组baPWV显著增高,分别为[14.70(9.17~21.39)m/s,12.86(10.00~15.24)m/s,P=0.025 1]。回波跟踪技术所测得的RA组的动脉僵硬度指标,即压力应变弹性系数(pressure elastic coefficient,E_ρ)和硬度系数(stiffness parameter,β)在肱动脉和胫后动脉较对照组增高,在胫后动脉与baPWV正相关(r=0.600 0,P<0.000 1;r=0.524 0,P<0.000 1) 结论 RA患者的动脉僵硬度较对照组显著增高,动脉硬化检测技术和超声回波跟踪技术相结合,能为临床提供早期、简捷、无创检测血管病变的方法。

【Abstract】 Objective To evaluate the peripheric arterial stiffness by echo-tracking technique and arteriosclerotic examine technique in patients with rheumatoid arthritis (RA).Methods Branchial-ankle pulse wave velocity (baPWV) was used in automatic detection in 25 RA patients and in 48 healthy individuals as a parallel control.At the same time, we studied the change of arterial stiffness and morphology with echo-tracking technique in all subjects.Results The baPWV in RA group significantly increased in comparison with control group [14.70(9.17~21.39) versus 12.86 (10.00~15.24)m/s,respectively;P=0.0251].Pressure elastic coefficient (E_ρ) and stiffness parameter (β) in patients with brachial artery and arterial tibialis posterior were significantly higher than those from normal subjects.E_ρ and β of arterial tibialis posterior were correlated positively with baPWV.Conclusions There is a significant increase of arterial stiffness in RA group.The combination of echo-tracking technique and arteriosclerotic examine technique could provide an early,simple,noninvasive way for vasculopathy testing.
