视网膜裂孔超声诊断特征分析
Diagnosis Characteristics in Retinal Tears by Ultrasonography
【摘要】 目的探讨视网膜裂孔的超声诊断特征及其诊断正确性。方法回顾分析86例(90只眼)超声诊断视网膜裂孔的超声声像图特征,并与临床眼底检查或眼内手术对照。结果 79例(83只眼)经临床眼底检查或眼内手术找到视网膜裂孔;7例(7只眼)未找到裂孔,其中4例临床确诊为玻璃体机化物,另外3例临床确诊为球壁玻璃体积血块黏附,超声诊断视网膜裂孔准确性为92.2%。视网膜裂孔主要声像图特征:玻璃体内见一短回声带,一端连于球壁,一端游离于玻璃体腔;玻璃体内见均匀膜状回声带,该回声带有中断区;玻璃体内见2条接近的短回声带,中间有中断区;单纯网膜回声带中断;玻璃体内见双层反折膜状回声带,呈发夹状。有以上其中1种超声表现即可诊断视网膜裂孔。结论超声对视网膜裂孔诊断正确性较高,可为眼科临床提供重要的信息。
【Abstract】 Objective To discuss the diagnosis characteristics and clinical value in retinal tears by ultrasonogra phy. Methods Retrospective analysis of the ultrasonogram characteristics of 86 patients with 90 eyes diagnosed as retinal tear by ultrasonography, and compared with the results obtained from fundus examination and ocular operation. Results 79 cases (83 eyes) were confirmed as retinal tears by fundus examination or ocular operation. 7 cases (7 eyes) were denied to have retinal tears, among which, 4 cases were diagnosed as vitreous fibroplasias and 3 cases as vitreous hemorrhage. The accuracy of ultrasonography for retinal tear was 92.2%. The main sonographic features of retinal tear. were: (1) Short echogenic band in the vitreous cavity, with one end at the wall of eyeball, and another end fluttered in the vitreous cavity. (2) Uniform membranous echogenic band in the vitreous cavity, with discontinuous echo region in the band. (3) Two adjacent short echogenic bands in the vitreous cavity, with discontinuous echo region between bands. (4) Retinal echogenic band interrupted. (5) Double-layer anti-fold membranous, hairpin like echogenic band in the vitreous cavity. Retinal tear can be diagnosed by ultrasonography with any one of the characteristics described. Conclusions Ultrasonography is valuable in diagnosis of retinal tear and favorable in the ophthalmic clinical practice.
