超声探头压力对中晚期妊娠胎儿大脑中动脉血流频谱的影响

【摘要】 目的探讨不同超声探头压力对中晚期妊娠胎儿大脑中动脉(MCA)血流频谱的影响。方法分别对50例胎儿近场及远场的MCA血流频谱进行检测以评价其相关性;然后,测量者1、2按相同步骤分别对50例胎儿进行MCA血流频谱检测,计算组内相关系数(ICC),评价测量者重复测量一致性(重复性)的信度;再将探头挤压胎儿头部使得头颅略有变形后再次测量,评价通过不同超声探头压力是否能对胎儿MCA血流频谱产生影响。结果近场及远场的MCA血流频谱检测,其相关性好;不同的检测者进行MCA血流频谱检测,其可重复性较好;探头对胎儿颅恼挤压前后比较,挤压后胎儿MCA-PI、MCA RI和MCA-Vs值升高,MCA-Vd减低(P<0.05)。结论中晚期妊娠胎儿MCA的血流频谱受探头挤压胎头的影响,应尽量减少探头对胎儿颅脑的压迫。

【Abstract】 Objective To investigate the influence on fetal middle cerebral artery (MCA) during the secondand -third-trimester pregnancy by the pressure of the acoustic probe. Methods The near-field and far-field MCA of 50 second-and-third trimester normal fetus were recorded with two operators recording the MCA of the 50 normal fetuses in the same order. Intraclass correlation coefficient (ICC) was engaged to evaluate the repeatability and agreement. And then pressed the fetal head to a little out of shape to evaluate the MCA again to investigate the influence on MCA of the fetus by the pressure of the acoustic probe. Results The repeatability of the near-field versus fa-field vessel and the different operators were almost perfect. After pressed, the fetus had an increased MCA-PI, MCA-RI and MCA-Vs, a decreased MCA-Vd (P<0.05) when compared to controls. Conclusions The MCA of the fetus during the second-and-third-trimester pregnancy could be influenced by the pressure of the acoustic probe, so we should avoid pressing the fetal head by the acoustic probe.
