实时三维经食管超声心动图的初步临床应用研究

【摘要】 目的探讨实时三维经食管超声心动图(RT3D-TEE)的临床应用价值。方法采用新型经食管三维探头对46例心脏病患者(其中心房颤动30例,房间隔缺损6例,瓣膜病10例)进行RT3D-TEE检查。旋转切割三维图像,从不同角度观察感兴趣区,对病变做出诊断。比较三维容积法和二维面积法测定的左心耳排空分数,并与左心耳最大排空速率进行相关性分析。结果 RT3D-TEE能快速清晰显示感兴趣区的病变形态特征,获得常规TEE无法得到的切面观,为诊断提供更直观准确的影像信息。三维容积法和二维面积法测定的左心耳排空分数有显著性差别(P=0.001),前者与左心耳最大排空速率相关性更好。结论 RT3D-TEE是一种安全可行的诊断技术,可作为常规TEE的有益补充,在临床上有重要应用前景。

【Abstract】 Objective To explore the clinical value of real-time three-dimensional (RT3D) transesophageal echocardiography (TEE) . Methods Forty six consecutive patients were performed RT3D TEE examinations using a new matrix array TEE probe, in which 30 with atrial fibrillation, 6 with atrial septal defect, 10 with valvular disease. Regions of interesting (ROI) were ohserved from different view by rotating and cropping the 3D images. Left atri al appendage (LAA) e iection fraction based on two-dimensional area measurements (EF_A) and three-dimensional volume measurements (EF_v) were calculated and related to LAA peak empty velocity (PEV) . Results RT3D-TEE pro vided fast and clear information about the pathomorphology of ROI, improved spatial orientation and provided some en-face views that can not be obtained by 2D TEE. EF_A was significantly higher than EF_v in atrial fibrillation patients (P=0. 001) . The correlation between EF_v and PEV (r=0. 655, P=0. 034) was better than that between EF_A and PEV (r=0. 536, P=0. 111) . Conclusions RT3D-TEE is a safe and feasible imaging modality that is complementary to conventional TEE. It has great potential for clinical applications.
