【摘要】 目的探讨甲状腺肿瘤中CerbB-2癌基因的表达与彩色血管能量成像(CPA)的相关性研究。方法术前应用CPA技术检测50例甲状腺癌患者及30例甲状腺腺瘤患者的血流信号。及其阻力指数(RI),术后采用免疫组化法检测该组织中CerbB-2癌基因的表达。结果 50例甲状腺癌CerbB-2癌基因的阳性表达率(70%)显著高于甲状腺腺瘤组(33.3%.P<0.05),cerbB-2在临床Ⅲ、Ⅳ期的阳性表达率(83.3%)显著高于Ⅰ、Ⅱ期(50%,P<0.05),伴有淋巴结转移的甲状腺癌cerbB-2癌基冈的阳性表达率为86·9%,尢淋巴结转移的CerbB-2的阳性表达率为45%,两者比较有统计学意义(P<0.05)。CerbB-2的表达强度与CPA血流分级呈正相关(γ=0.699,P<0.05),甲状腺癌的RI值(0.69±0.13显著高于甲状腺腺瘤组的RI值(0.54±0.17.P<0.05)。CerbB-2的表达强度与RI值呈正相关(γ=0.792,P<0.05)。结论 CerbB-2癌基因在甲状腺癌中呈高强度表达,其表达强度与CPA血流的分级呈正相关,两者的有机结合可在术前、术后对甲状腺肿瘤进行评估,为临床早期诊断和评估预后提供新的理论依据。更多还原

【Abstract】 Objective To assess the relationship between the color power angioimaging (CPA) of blood flow and the expressions of CerbB-2 cancer gene in thyroid tumor. Methods The blood signal and resistance index (RI) of fifty patients with thyroid carcinoma and thirty cases with adenomas were measured preoperatively by CPA whereas the expressions of their CerbB-2 cancer gene were examined with immunohistochemistry after operation. Results The positive expression rate (70%) of CerbB-2 in the group of thyroid carcinoma was higher than that in adenomas group (33.3%, P<0.05) .For thyroid carcinoma, the CerbB-2 positive expression rate was 83.3% in clinical stages Ⅲ- Ⅳ, which was significantly higher than that in the stages of Ⅰ-Ⅱ (50%, P<0.05) . The positive rate of CerbB-2 expression of thyroid carcinoma with lymph nodes metastasis was 86.9% while it was 45% in the group without nodes metastasis. The comparison between these two groups revealed a significant difference. The CerbB-2 intensity in thyroid carcinoma demonstrated a positive correlation to the CPA degrees (γ=0. 699, P<0.05) . The RI value of malignant group (0.69±0. 13) was slight higher than that of the adenomas group (0.54±0. 17, P<0.05) . The intensity of CerbB-2 correlated positively to the RI values (γ=0. 792, P<0.05) . Conclusions High expressions of CerbB-2 cancer gene were observed in thyroid carcinoma and the expression intensity correlated positively to the CPA degrees. The integration of CerbB-2 and CPA may evaluate the patients’ conditions before and after the operation of thyroid carcinoma, hence provides information important for the earlier diagnosis and judgment of prognosis.更多还原
