【摘要】 目的:观察经阴道注水腹腔镜(Transvaginal hydrolaparoscopy,THL)在不孕症诊治中的临床效果和安全性。方法:对53例不孕症患者进行THL检查和治疗,记录检查和治疗结果、术中后穹隆穿刺成功率、转行标准腹腔镜(LAP)率、与LAP比较的诊断符合率、并发症发生率、手术时间、门诊留观时间、手术费用等。结果:检查发现异常33例,术中经输卵管通液术或宫腔镜下输卵管插管通液术疏通阻塞输卵管11例;穿刺成功率为92.45%;转行LAP率为32.0%,THL与LAP诊断符合率为91.6%;THL手术并发症为1.9%。平均手术时间、门诊留观时间和手术费用分别为(24.9±4.5)min、(128.6±23.9)min和(739±97)元。结论:THL在不孕症诊治中能提供与LAP准确度相似的结果,操作简单、准确、安全、微创、经济,可作为不孕症早期检查及治疗的手段。更多还原

【Abstract】 Objective:To observe the clinical efficacy and safety of transvaginal hydrolaparoscopy (THL) in diagnosis and treatment of infertility.Methods:53 cases with infertility underwent THL,the results,success rate of culdocentesis and the rate of conversion to transabdominal laparoscopy were recorded,the coincidence rate of diagnosis,incidence of complications,operation time,discharge time and cost were compared between THL and transabdominal laparoscopy.Results:33 cases were found with abnormality,the tubes of 11 cases were unobstructed after hydrotubation or hydrotubation under hysterscope; the success rate of culdocentesis was 92.45%; the rate of conversion to transabdominal laparoscopy was 32.0%,the coincidence rate of diagnosis between THL and transabdominal laparoscopy was 91.6%; the incidence of complications was 1.9%.The mean operative time,discharge time and cost were (24.9±4.5) minutes,(128.6±23.9) minutes and (739±97) yuan,respectively.Conclusion:The accuracy of THL in diagnosis and treatment of infertility is similar to that of transabdominal laparoscopy,THL can be used as an early examination and treatment method with the advantages of simpleness,accuracy,safety,minimal invasion and economy.更多还原
