【摘要】 目的:探讨膜式液基薄层细胞学技术(Thinprep cytology test,TCT)在卵巢恶性肿瘤腹水/腹腔冲洗液细胞学诊断中的临床应用价值。方法:收集2002年1月～2006年12月卵巢恶性肿瘤患者腹水/腹腔冲洗液标本124例,同时采用传统普通涂片方法和新柏氏薄层细胞学检测系统进行细胞学检查,对结果进行统计分析。结果:TCT涂片细胞结构及背景清晰、细胞单层分布,无重叠;在细胞量、异常细胞量及背景材料方面,TCT涂片优于传统普通涂片;124例标本中新柏氏TCT法检测腹水/腹腔冲洗液瘤细胞阳性39例(31.45%),可疑癌12例(9.68%),阴性73例(58.87%),而传统普通涂片方法检测出瘤细胞阳性18例(14.52%),可疑癌32例(25.81%),阴性74例(59.68%)。两种制片方法的恶性细胞确诊率差异有统计学意义(P<0.05)。结论:TCT细胞学检查法在标本的处理、制片和诊断明显优于传统的涂片方法,能够提高卵巢恶性肿瘤腹水瘤细胞分布的集中程度,提高瘤细胞诊断率,为临床治疗提供可靠的诊断依据。更多还原

【Abstract】 Objective:To explore the clinical application value of Thinprep cytology test(TCT) in cytologic diagnosis of ascites and peritoneal washings in patients with malignant ovarian tumor.Methods:124 samples of ascites and peritoneal washings in patients with malignant ovarian tumor were collected from January 2002 to December 2006,then cytologic test was carried out by traditional cells smears and TCT,and the results were analysed.Results:The structure and background of cells were distinct,the distribution of cells was monodermic,without overlap;in terms of cell amount,abnormal cell amount and background,TCT was superior to traditional cells smears;39 cases(31.45%) of malignant ovarian tumor,12 cases(9.68%) of suspicious cancer and 73 negative cases(58.87%) were detected by TCT;while 18 cases(14.52%) of malignant ovarian tumor,32 cases(25.81%) of suspicious cancer and 74 negative cases(59.68%) were detected by traditional cells smears.There was significant difference in diagnostic rate of malignant cells between the two methods(P<0.05).Conclusion:In terms of handing of samples,slice production and diagnosis,TCT is superior to traditional cells smears,which may improve the intensive grades of distribution of oncocytes in ascites,increase diagnostic rate of oncocytes,and provide a basis for clinical treatment.更多还原
