甲强龙、电针联合AECs移植治疗脊髓损伤的实验研究

【摘要】 目的探讨甲强龙、电针与羊膜上皮细胞(amniotic epithelial cell,AECs)移植联合治疗,对脊髓损伤(spinal cord injury,SCI)大鼠下肢运动功能的影响。方法将60只成年雌性Wistar大鼠随机分成5组,每组12只。A组(SCI损伤对照):做SCI手术,不进行治疗;B组(甲强龙治疗):SCI后,用大量甲强龙药物冲击治疗,共3d;C组(MP+电针):B组基础上,SCI后4h,行华佗夹脊穴电针治疗;D组(MP+电针+AEC):C组基础上,SCI后第7天,在脊髓损伤处移植大鼠AECs;E组(假手术):只打开椎板,暴露脊髓,不造成SCI。各组定期行为学观察(BBB评分),术后30d行5-HT免疫荧光组织化学观察和神经电生理检测。结果 5-HT染色:D组损伤区可见大量有序的5-HT阳性神经纤维,与E组最接近;BBB评分:D组恢复最为明显,与其它治疗组比较差异显著;MEP检测:D组峰-峰值显著增加,潜伏期明显缩短,差异有统计学意义(P<0.01)。结论甲强龙、电针与AECs联合治疗脊髓损伤极大的促进了5-羟色胺能神经纤维的再生,对SCI大鼠后肢运动功能的恢复有明显的促进作用。

【Abstract】 Objective To explore the combined effect of methylprednisolone (MP),electro-acupuncture and amniotic epithelial cell (AECs) transplantation on hind limb motor function for rats with spinal cord injury.Methods Sixty female Wistar rats were randomly assigned to 5 groups (n=12 per group):Group A (SCI control) accepted no treatment after SCI created, Group B (MP treatment) accepted MP pulse treatment after SCI for 3 days, Group C (MP+Electro-acupuncture) accepted electro-acupuncture treatment on Huatuo Jiaji acupoint 4h after SCI based on group B,Group D (MP+Electro-acupuncture+AECs) accepted AECs transplantation treatment in SCI region 7d after SCI base on group C,Group E (Sham operation) with neural scute opened and spinal cord was exposed without SCI. Ethological observation (BBB scores) was done regularly. 5-HT immunofluorescence histochemistry and electrophysiological tests were done on 30d after operation.Results 5-HT staining resulted in SCI region of Group D contained numerous ordinary 5-HT-positive axons,most approximate to group E.BBB scores revealed that functional recovery was best in group D.Significant difference lay between group D and other therapy groups.MEP demonstrated that peak-to-peak value increased sharply and latency was shortened obviously in group D.Difference between group D and other SCI groups was statistically significant(P<0.01).Conclusion A combination of methylprednisolone,electro-acupuncture and AECs transplantation can promote the regeneration of 5-HT containing axons and the recovery of hind limb motor function for SCI rats.
