屈肌腱不同吻合方法的生物力学测试及比较

【摘要】 目的对三种最基础的肌腱缝合方法的力学性能进行测量和比较,从生物力学的角度了解三种方法中哪种更能满足早期功能锻炼的需要,更能有效防止肌腱再断裂和吻合处间隙形成。方法取猪的趾深屈肌腱共24条,随机分成三组,游离切断后分别用双"+"字缝合法、改良Kessler法、双股Tsuge法进行缝合;用生物力学测量机对缝合后的肌腱分别进行生物力学测验,测验内容包括2mm间隙形成负荷、最大负荷、断裂功耗及循环负荷试验(疲劳实验),将测量结果进行统计学分析。结果三组肌腱吻合方法的2mm间隙形成负荷,最大负荷,三者的断裂功耗,及周期负荷实验能耐受的循环次数为:双"+"字缝合法>改良Kessler法>双股Tsuge法,三组之间差异有显著性意义(P<0.01)。结论 (1)所测的三种肌腱缝合方法中,双"+"字缝合法是一种简单易行而强度又高的缝合技术,在牵拉实验过程中不易形成间隙,不易再断裂。改良Kessler缝合法的力学性能居中,双股Tsuge法的力学性能最差。(2)在周期负荷实验中,双"+"字缝合法表现出良好的适应周期载荷的柔韧性。

【Abstract】 Objective To test and compare mechanical properties of Flexor tendons sutured in three different elementary methods,and make certain which method is the best to meet the requirement of earlier functional exercise from the angle of biomechanics and prevent effectively re-fragmentation of tendon and suture gap formation.Methods 24 porcine flexor digitorum profundus tendons were randomly divided into three groups,Tendons of each group were repaired by one of the following suture techniques:double-cross method,the modified Kessler and the Tsuge method.And then biomechanics of differently sutured tendons were tested by biomechnical machine,the testing contents including 2mm gap formation force,peak load,energy to break point,and cyclic tension testing.The results were analyzed statistically.Results In the group of using double-cross suture technique,the 2-mm gap formation force,the peak load,and the energy to break point were much higher than that using the modified Kessler technique and the Tsuge technique.The cyclic tension testing found the double-cross suture technique to be superior to the other methods under cyclic loading.There were significant difference among the three suture techniques(P<0.01).Conclusion(1)In three kinds of suture techniques we had tested,the double-cross suture technique was of the first choice because it was easy to operate,it can offer reliable tensile strength,fight against gap formation in tension test.(2)In cyclic tension testing,the double-cross suture technique shown the pliability that can adapt cyclic loading.
