【摘要】 目的评价经尿道前列腺等离子双极电切(TUPKRP)联合气压弹道/超声碎石术治疗良性前列腺增生症(BPH)合并膀胱结石的临床效果。方法2007年3月至2009年4月,采用TUPKRP联合气压弹道/超声碎石术治疗BPH合并膀胱结石患者86例。结果86例均手术成功,膀胱结石均一次性清除;手术时间45～150min,平均89min,其中膀胱结石清除时间为11～48min,平均29min;术中无膀胱破裂、严重失血、电切综合征等严重并发症发生;术后4～5天拔除导尿管,其中79例排尿通畅,最大尿流率(Qmax)>15ml/s;7例排尿不畅,Qmax<6ml/s,给予膀胱造瘘、药物治疗和膀胱训练2～3个月后排尿通畅。结论TUPKRP联合气压弹道/超声碎石术治疗BPH合并膀胱结石安全有效,手术操作简单,术后并发症少,可避免开放手术所致的伤口感染,是治疗BPH合并膀胱结石的理想方法。更多还原

【Abstract】 Objective To evaluate the efficacy and safety of transurethral bipolar plasmakinetic resection of the prostate (TUPKRP) combined with pneumatic and ultrasound lithotripsy in the treatment of benign prostatic hyperplasia (BPH) with bladder stones.Methods From March 2007 to April 2009,TUPKRP combined with pneumatic and ultrasound lithotripsy was performed in 86 patients with BPH and bladder stones.Results The operation was successful in all 86 cases without complication,with bladder stones completely removed.The operative time was 45 ~ 150 min (89 min on the average),including bladder stone removal time of 11 ~ 48 min (29 min on the average).Urethral catheters were removed from the patients 4 to 5 days after operation,while 79 cases voided fluently (Qmax>15 ml/s) and 7 cases with postoperative dysuria (Qmax<6 ml/s).Satisfactory result (Qmax>15 ml/s) was gained after given cystostomy,drug therapy and 2~3 month bladder training.Conclusion It is suggested that TUPKRP combined with pneumatic and ultrasound lithotripsy is a safe and effective way in the treatment of BPH with bladder stones for its simple operation,less post-operative complications and avoidance of wound infection due to open surgery.更多还原
