【摘要】 目的回顾调查了解影响肺癌住院费用的主要因素,为有效控制医疗费用的不合理增长提供政策依据。方法抽样调查住院接受手术治疗的肺癌病例190例,对2家医院住院费用、患者年龄、性别、付费方式、住院次数、入院合并症、术后抗生素使用、抢救、转归、分期、手术时间、术前住院天数以及住院天数等可能影响住院费用的因素进行t检验、方差分析和多元逐步回归分析。结果 2家医院间自费患者、住院次数、手术时间、术前住院天数和住院天数间存在显著性差异,而影响住院费用的因素主要有病例来源、术前住院天数、住院次数和住院天数。住院费用中西药费的比例偏大,而体现手术技术含量的项目收费偏低。结论完善医疗保险制度,积极实施临床路径,健全早诊早治机制,是有效控制住院费用的重要措施。更多还原

【Abstract】 Objective To explore retrospectively the main factors influencing the hospitalization expenses of lung cancer so as to provide policy suggestions for the control of irrational increase of medical expenses.Methods The data of lung cancer (190 inpatients were selected at random) were collected.T test, ANOVA and multiple stepwise regression analysis were used to screen age, sex, type of payment, number of admissions, complications, antibiotics used postoperation, savage, condition of leaving hospital, stage, operating time, days of preoperation, hospital stay and so on, to identify the major factors influencing the hospitalization expenses.Results There are significant differences of patients’ at their own expense, number of admissions, operation time pre-operative time and hospital stays.The main influencing factors of the hospitalization expenses including cases resource, days of preoperation, the length of stay and number of admissions.The high proportion of Chinese and Western medicine costs compared with the low proportion of operationd technological item costs.Conclusion Health care system should be perfected.The protocol for the treatment of lung cancer should be developed.Mechanism of early diagnosis is the key to control hospitalization expenses.更多还原
