MRI动态增强扫描在垂体微腺瘤诊断中的价值

【摘要】 目的:评估MRI动态增强扫描的检查质量和其在垂体微腺瘤诊断中的价值。方法:回顾性分析48例经临床及实验室检查诊断为垂体微腺瘤的MRI平扫、Gd-DTPA动态增强扫描及常规延迟增强扫描图像,判断Gd-DTPA动态增强扫描图像质量,以及统计MRI平扫、Gd-DTPA动态增强扫描及常规延迟增强扫描检出率。结果:MRIGd-DTPA动态增强扫描成功率为100.0%,MRI平扫、动态增强扫描及常规延迟增强扫描对病灶检出率分别为43.8%(21/48)、91.7%(44/48)、66.7%(32/48)。三种不同的扫描方法病灶检出率均有显著性差异(均P<0.05)。结论:MRIGd-DTPA动态增强扫描是相对简单、易行、成功率高的检查方法,可以显著提高垂体微腺瘤的检出率,可作为MRI诊断垂体微腺瘤的常规检查方法。

【Abstract】 Objective:To evaluate the examinational quality and the values of dynamic contrast enhancement MRI in the diagnosis of pituitary microadenomas.Methods:48 cases image with pituitary microadenomas confirmed by clinical manifestation and biochemical analysis were studied by plain MRI scan,Gd-DTPA dynamic enhanced MRI and conventional delayed enhanced MRI,evaluated the quality of image of Gd-DTPA dynamic enhanced MRI,and the rate of lesion detec tion of plain MRI scan,Gd-DTPA dynamic enhanced MRI and conventional delayed enhanced MRI were analyzed.Results:The rate of success in Gd-DTPA dynamic enhanced MRI was 100.0%,the positive rate of diagnosis for pituitary microadenomas with plain MRI scan,dynamic enhanced MRI and conventional delayed enhanced MRI was 43.8%(21/48),91.7%(44/48) and 66.7%(32/48),and there were statistically significances (P<0.05) between different scanning methods.Conclusion:Gd-DTPA dynamic enhanced MRI is an examinational method of relative simple,easily carry,the highly rate of success.And obviously improve the rate of lesion detection of pituitary microadenomas.Dynamic contrast enhancement MRI should be considered as the conventional examination in pituitary microadenomas.
