【摘要】 目的皮肤淋巴瘤是较常见的结外淋巴瘤,本文初步探讨PET/CT在皮肤淋巴瘤中的价值。方法回顾性分析我院近5年来的12例皮肤淋巴瘤患者PET/CT图像,根据治疗情况分为治疗组(A组)及未治疗组(B组),比较皮肤病变与PET/CT结果的关系。结果 A组中6例行PET/CT随访及再分期,其中2例PET/CT显像未见FDG高代谢病变;3例患者复发的皮肤病变具有不同程度的FDG摄取,1例患者肺部转移。B组中6例皮肤淋巴瘤PET均有阳性病变,但2例患者皮损或皮下结节无FDG摄取。综合分析10例PET/CT显像阳性者,8例患者有皮肤红斑和皮下结节,其中2例皮肤病变未见FDG摄取,2例仅部分皮肤病变摄取FDG。10例分期及再分期的淋巴瘤中,与常规方法比较PET/CT使2例(20%)分期上调。结论 FDGPET/CT对皮肤淋巴瘤的诊治有一定的临床价值。更多还原

【Abstract】 Objective Cutaneous lymphoma is the common group of extranodal non-Hodgkin lymphomas.18F-fluorodeoxyglucose(FDG) positron emission tomography(PET/CT) have been widely used in lymphoma.This research was to investigate the usefulness of FDG PET/CT in the management of cutaneous lymphoma.Methods The 12 patients with cutaneous lymphoma [nine T/NK cell non-Hodgkin lymphoma(NHL) and three B-cell NHL] were examined by PET/CT retrospectively.There were 9 men and 3 women(age range 16-79 years) who were performed PET/CT for initial staging and restaging following therapy.The patients were divided into 2 groups.Group A with 6 cases who had performed therapy before PET/CT scans,and Group B with 6 cases without therapy.All cases were followed up at least 6 months.Results Among group A,the 6 cases had performed 8 PET/CT scans,2 of 6 cases with negative PET/CT scan had not relapsed with follow-up more than 6 months.Four cases had positive PET/CT scans post-therapy.In group B,the 6 cases had performed with positive PET/CT results,however,2 of 6 cases with cutaneous lesions displayed negative FDG.Combined group A and B with positive PET/CT scans,there were cutaneous lesions in eight patients by visual inspection,however,2 patients with negative FDG PET/CT and another 2 patients with positive FDG PET/CT of partial cutaneous lesions;FDG PET had a sensitivity of 75% for the evaluation of cutaneous lesions by patients.For staging of cutaneous lymphoma,two patients were upstaged by FDG PET/CT compared with conventional methods.Conclusion FDG PET/CT has a potential value for initial staging and evaluating therapy reaction in patients with cutaneous lymphoma.更多还原
