尼莫地平治疗重度迟发型Vit K缺乏性颅内出血后神经损害的临床研究
Clinical Study of Nerve Injury of Nimodipine Treatment of Severe Late-onset Vit K Deficiency with Intracranial Hemorrhage
【摘要】 目的研究尼莫地平在治疗重度迟发型Vit K缺乏性颅内出血后神经损害的影响。方法回顾性分析2001年06月~2007年10月入住我科的50例重度迟发型Vit K缺乏性颅内出血患儿。将50例患儿随机分为治疗组和对照组,采用双盲试验进行临床病例对照研究。对照组仅给予一般的支持和对症处理;治疗组除一般常规治疗外,加用尼莫地平0.2~0.5ug/kg/min持续泵维,五天后改为尼莫地平1~2mg/kg/d分两次口服。结果对照组25例患儿经一般治疗后,其中8(32%)例患儿其神经损害获得了早期显效,6(24%)例患儿有效,11(44%)例患儿无效;10(40%)例患儿获得远期显效,8(32%)例患儿有效,7(28%)例患儿无效。治疗组25例患儿经尼莫地平治疗后,其中18(72%)例患儿其神经损害获得了早期显效,6(24%)例患儿有效,1(4%)例患儿无效;20(80%)例患儿获得了远期显效,5(20%)例患儿有效。结论尼莫地平能明显降低神经系统的损害。
【Abstract】 Objective Study of the impact of nimodipine on the nerve damage after the severe late-onset intracranial hemorrhage with vitamin K deficiency.Methods A retrospective analysis in June 2001 ~ October 2007 ,children admitted in our department a Section 50 cases of severe delayed-type vitamin K deficiency in children with intracranial hemorrhage.50 patients were randomly divided into treatment group and control group,double-blind clinical trial conducted case-control study.The control group were given general support and symptomatic treatment;treatment group addition to the normal routine treatment,nimodipine was added 0.2 ~ 0.5ug/kg/min continuous pump-dimensional,five days later changed to nimodipine 1 ~ 2mg/kg / d in two oral.Results The control group,25 cases of children after general treatment,of which 8 (32%) cases of neurological damage in children received their early markedly,6 (24%) cases of children with effective,11 (44%) cases of children with valid ;10 (40%) cases of children with access to long-term markedly,8 (32%) cases of children with effective,7 (28%) cases of children is invalid.Treatment group,25 cases of children after nimodipine treatment,of whom 18 (72%) cases of neurological damage in children received their early markedly,6 (24%) cases of children with effective,1 (4%) cases of children with invalid;20 (80%) cases of children with access to a long-term markedly,5 (20%) cases of children effectively.Conclusion Nimodipine can significantly reduce the damage to the nervous system.
