【摘要】 目的:探讨肾透明细胞癌的MRI表现,提高对该病的认识。方法:搜集临床资料完整并经手术病理证实的肾脏透明细胞癌25例,均行MRI扫描,其中16例行MRI增强扫描。结果:本组25例患者均为单肾发病,其中左肾11例,右肾14例。肿块直径2.0～16.2cm,其中肿块直径小于4.0cm8例,直径大于4.0cm17例。平扫在T1WI呈低信号20例,等信号2例,混杂信号3例,T2WI呈高信号22例,低信号1例,混杂信号2例。肿块呈膨胀性生长21例,呈浸润性生长4例。肿块内见不规则坏死、囊变区,MR信号显示不均12例。显示假包膜12例。增强显示16例均以边缘强化为主,全瘤均呈不均匀强化;16例中于皮质期明显强化4例,强化程度高于肾皮质,呈轻中度强化11例,不强化1例;实质期明显强化3例,轻度强化13例;肾盂期16例均呈轻度强化。12例肿块内有不规则坏死、囊变区于增强各期中均未见强化。假包膜呈轻中度持续性强化。结论:MRI能观察肾癌生长方式,了解侵犯周围组织及转移情况,根据信号和增强表现能作出较正确诊断。更多还原

【Abstract】 Objective:To investigate the MRI performance of renal clear cell carcinoma,and improve the understanding of the disease.Methods:Collected 25 patients that with renal clear cell carcinoma confirmed by surgery and pathology that with complete clinical data,all the 25 patients were performed plain MRI scans,16 of them performed enhanced MRI scans.Results:All the 25 patients in this group were single lesions,11 lesion in left kidney,14 lesion in right kidney.Lesion maximum diameter of 2.0 to 16.2 cm,of which 8 cases of lesion diameter less than 4.0 cm,8 cases of lesion diameters larger than 4.0 cm.Unenhanced MRI T1WI showed low signal in 20 cases,moderate signal in 2 cases,mixed-signal 3 cases,T2WI showed high signal in 22 cases,low signal in 1 case,mixed-signal in 2 cases.Tumor showed expansive growth in 21 cases,showing invasive growth of 4 cases.12 cases of lesions,see irregular necrosis,cystic areas,MR signals uneven.Pseudocapsule was found in 12.All 16 cases of enhanced mainly showed edge enhancement,the entire tumor showed unevenly enhancement;Cortical phase:4 cases obviously enhanced,and the degree of enhancement is higher than the renal cortex,11 cases were mild to moderate enhancement,one cases had no obvious enhancement;Renal parenchymal phase:3 cases obviously enhanced,13 patients with mild enhancement;Renal sinus phase:all 16 cases showed uniform slight enhancement.There are 12 cases of irregular tumor necrosis,cystic areas,and necrosis,cystic areas in all phases were no significant enhancement.12 cases pseudocapsule were mild to moderate enhancement.Conclusion:MRI can show the growth pattern of renal clear cell carcinoma,also can understand the invasion of the surrounding tissue and distant metastasis,according to plain and enhanced MRI performance can make a more correct diagnosis.更多还原
