【摘要】 目的:探讨超声吸引刀(CUSA)结合单极电凝在肝切除中的应用价值。方法:251例需行肝脏切除术的患者中运用CUSA结合电凝(CUSA组)95例,运用钳夹法(CC组)156例,比较两组手术时间、术中出血量及输血例数、住院时间、术后肝功能及并发症发生情况。结果:与CC组相比,CUSA组肝门阻断率低、手术时间较长、住院时间短(P均<0.05),两组术中出血量、输血例数比较差异无统计学意义(P>0.05)。另CUSA组患者术后ALT、总胆红素水平较CC组低(P均<0.05),术后并发症发生率及手术死亡率差异无统计学意义(P>0.05)。结论:运用CUSA结合单极电凝行肝脏切除术对肝组织损害轻,术后恢复快,术后并发症不增加,不足之处是手术时间延长。更多还原

【Abstract】 Objective:To investigate the clinical value of cavitron ultrasonic surgical aspirator (CUSA) combining monopolar electrocoagulation in hepatectomy. Methods:Two hundred and fifty-one patients accepted hepatectomy were enrolled,CUSA combining monopolar electrocoagulation (CUSA group) was used in 95 patients and clamp crushing method (CC group) was used in 156 patients. The operative time,blood loss during operation,the number of cases receiving blood transfusion,hospital stay,postoperative liver function and postoperative complications were observed. Results:Compared with CC group,CUSA group had lower blocking rate of porta hepatis,longer operative time,shorter length of hospital stay,lower level of ALT and otal bilirubin. The blood loss during operation,patients requiring blood transfusion,rate of postoperative complications and mortality had no significant difference between the two groups. Conclusion:CUSA combining monopolar electrocoagulation allows hepatectomy with the advantage of relieving postoperative liver injury and reducing hospital stay while,postoperative complications don’t increase,but the only disadvantage may be a longer operative time.更多还原
