慢性前列腺炎伴不育症患者前列腺液中病原学检查及结果分析

【摘要】 目的研究慢性前列腺炎与病原菌的关系,探讨前列腺液病原菌检测的诊断价值及其临床意义。方法对临床慢性前列腺炎患者的前列腺液进行细菌培养和药敏试验,同时检测沙眼衣原体和解脲脲原体。结果 227例标本中有127例分离出细菌,阳性率55.9%,革兰阳性菌占74.8%,以表皮葡萄球菌属为主;革兰阴性菌占21.3%,以大肠埃希菌为主;合并支原体和(或)沙眼衣原体感染43例,占18.9%。100例细菌培养阴性标本中,支原体阳性47例(47%),沙眼衣原体阳性25例(25%),其中两者合并存在8例(8%);分离出的细菌对临床常用抗菌药物的耐药率较高,对β-内酰胺类、氨基糖苷类、复方新诺明、利福平大部分敏感,耐药率7.4%~18.9%;对青霉素类耐药率最高,为68.7%~100.0%,但对万古霉素全部敏感。解脲支原体对环丙沙星耐药率最高(54.4%);对交沙霉素、四环素耐药率最低(2.9%)。人型支原体对红霉素的耐药率最高(100.0%);对强力霉素、磷霉素全部敏感(100.0%)。结论慢性前列腺炎伴不育症患者进行前列腺液中分离菌株对不同抗菌药物耐药性有很大差异,适时联合检测病原菌及药敏试验对临床诊断与治疗具有重要的意义。

【Abstract】 Objective To investigate the relationship between chronic prostatitis and the pathogenic bacteria in infertile men,and study the diagnostic value and clinic significance of pathogenic diagonosis in prostatic fluid.Methods Bacterial culture and antimicrobial agents sensitivity test were applied to prostatic fluid in 227infertile patients with chronic prostatitis as well as mycoplasma and Chlamydia were detected.Results Bacteria were found 127patients out of 227(55.9%).Gram-positive bacteria were about 74.8%and Staphylococcus epidermidis was the most predominant.Gram-negative bacteria were about 21.3%and Escherichia coli was the most common.Forty three patients were infected by C.trachomates and Mycoplasma(18.9%).In 100cases of bacterial culture negative specimens,47cases were infected with Mycoplasma,accounting for 47%,Chlamydia trachomatis infections in 25cases,accounting for 25%,both of infection in 8cases,accounting for 8%.According to sensitivity test,the ratio of drug resistance of bacteria from prostatic fluid was high,the highest against penicillins(68.7%~100.0%);for the lowest against β-lactams,aminogly-cosides,trimethoprim+sulfamethoxazole,rifampin and vancomycin(7.4%~18.9%),but their antimicrobial resistance was the lowest against vancomycin(0.0%);for the Ureaplasma urealyticum,it was the highest against ciprofloxacin(54.4%),and the lowest against josamycin,tetracycline and fosfomycin(2.9%);for the Mycoplasma hominis,it was the highest against erythromycin(100.0%),and the lowest against doxycycline and fosfomycin(0.0%).Conclusion Isolated bacterial strains differ greatly in their resistance against different antibiotics on prostatic fluid of infertile men with clinical prostatitis.Timely detection of pathogens and susceptibility testing is important for clinical diagnosis and treatment of infertile men with clinical prostatitis.
