冠心病的严重程度与心肺功能相关性分析

【摘要】 目的:用心肺运动试验中的氧摄取动力学、无氧阈及心肺储备功能对冠状动脉狭窄的严重程度进行评估。方法:对68例胸痛的患者行心肺运动试验,彩色心超测定左心室射血分数(LVEF),及冠心病造影,根据冠状动脉主要血管的狭窄程度进行分组,狭窄程度<50%(29例),50%~70%(25例),>70%(14例),组间进行二氧化碳呼吸当量、氧摄取动力学及无氧阈、LVEF等的比较。结果:与<50%组相比,50%~70%组的中VO2peak[(20.86±6.36)vs.(26.63±4.51)mL.kg-1.min-1,P<0.001]、VCO2[(1.37±0.50)vs.(1.72±0.49)L/min,P<0.05]、AT[(1.24±0.38)vs.(1.48±0.32)L/min,P<0.05]、O2pulsepeak[(10.34±3.27)vs.(12.33±2.53)mL/beat,P<0.05]有差异有统计学意义,<70%组中的,除了VO2peak、VCO2、AT、O2pulsepeak有显著性差异,HRpeak[(120.50±14.00)vs.(139.03±18.97)beats/min,P<0.01]、VE/VCO2@AT[(31.23±4.89)vs.(28.88±4.04),P<0.05]、VD/VT(Est)@Peak[(0.21±0.05)vs.(0.17±0.03),P<0.01]同时也有有显著性差异;与50%~70%相比,>70%组中的VD/VT(Est)@Peak有显著性差异。结论:冠状动脉狭窄程度大于50%可能与VO2peak、VCO2、AT、O2pulsepeak有一定的相关性,冠状动脉狭窄程度>70%可能与HRpeak、VE/VCO2@AT、VD/VT(Est)@Peak有一定的相关性。

【Abstract】 Objective:The objective of this study was to investigate the effect on evaluation of stenosis severity of coronary artery with the kinetics of oxygen uptake(VO2),anaerobic threshold(AT)and cardiopulmonary functional capacity during the onset of exercise in patients with suspected angina.Methods:Treadmill graded exercise tests with gas analysis 、ultrasound cardiography、coronary angiography were conducted on sixty-eight patients with suspected angina.According to stenosis severity of coronary artery,sixty-eight patients were divided into three groups:less than 50% stenosis(39),50%-70% stenosis(29),and more than 70% stenosis(14).Evaluate the differences of cardiopulmonary function between groups.Results:Compared with less than 50% stenosis group,50%-70% stenosis group showed statistical significance in VO2peak、VCO2、AT、O2 pulse peak,more than 70% stenosis group not only shown showed statistical significance in VO2peak、VCO2、AT、O2 pulse peak,but also showed statistical significance in HR peak[(120.50±14.00)vs.(139.03±18.97)beats/min,P<0.01]、 VE/VCO2@AT[(31.23±4.89)vs.(28.88±4.04),P<0.05] and VD/VT(Est)@Peak[(0.21±0.05)vs.(0.17±0.03),P<0.01].Compared with 50%-70% stenosis,more than 70% stenosis group showed statistical significance in VD/VT(Est)@Peak.Conclusions:CEPT have potential value in evaluating stenosis severity of coronary artery.HR peak、 VE/VCO2@AT and VD/VT(Est)@Peak have some connection with the severity of coronary heart disease.
