【摘要】 目的探讨妇科疾病合并糖尿病者围手术期处理措施。方法将2005年1月～2008年6月收治的134例合并糖尿病的妇科手术病人,根据围手术期空腹血糖控制程度不同分为两组,A组69例,术前、术中血糖控制为7.1～11.1mmol/L,术后血糖≤7.0mmol/L。B组65例,围手术期血糖控制为4.0～7.0mmol/L,进行回顾性分析。结果 134例全部痊愈出院。两组未发现明显酮症酸中毒和高渗性昏迷者。围手术期A组发生较为严重低血糖反应1例,冠心病发作2例,共3例。B组发生上述并发症分别为7例和5例,共12例。两组比较p<0.01,有非常显著性差异。两组发生肺部感染4例,泌尿系感染7例,皮肤伤口2～3期愈合9例,但无显著性差异。结论妇科疾病合并糖尿病者术前、术中应保持轻度高血糖,术后血糖为中等或偏低水平,对降低较严重低血糖反应和冠心病发作并发症率有一定意义。早期加强抗炎等综合治疗对预后有益。更多还原

【Abstract】 Objective To discuss an ideal set of treatments for gynecological patients with diabetes during perioperative period.Methods Gynecological patients hospitalized around 2005~2008,who were of diabetes and required gynecology operation,were divided into two groups.Group A consisted of 69 patients,whose blood glucose level before and during operation were controlled between 7.1~11.2 mmol/L,and that after operation less than 7 mmol/L.Group B consisted of 65 patients,whose blood glucose during perioperative period was between 4~7.0 mmol/L.Results 134 patients above were totally cured and discharged,and none of them suffered from ketoacidosis or hyperosmotic coma.Group A involved a case of hypoglycemia reaction and two cases of coronary heart disease while Group B involved respectively 7 cases and 5cases.Result of comparison between the two groups shows p<0.01,which indicated significant differences existed between the two groups.Both two groups were referred to infection,with 7cases related to respiratory system,6 cases to urinary system and 8 cases to wounds of skin.But the infectious complication didn’t add any significant difference towards the two groups.Conclusion It’s beneficial to adequately elevate the blood glucose level before and during operation while to cautiously control it among the moderate level after operation,for it can decrease the risk of hypoglycemia reaction and other possible complications.To emphasize anti-inflammation treatment also contributes to the prognosis of the patients.更多还原
