【摘要】 目的探讨Ⅰ期人工全髋关节置换术治疗有移位的老年股骨颈骨折的临床效果。方法采用髋关节后外侧改良Gibson入路对有移位的老年股骨颈骨折32例32髋行Ⅰ期人工全髋关节置换。结果32例均获随访,平均时间23.4(12~36)个月。1例术后出现伤口脂肪液化,经清创及伤口换药后愈合;2例出现一侧下肢轻度肿胀,考虑为深静脉血栓所致,予抗凝、抬高患肢等对症处理后肿胀消退。无关节感染、肺部感染等早期并发症,亦未发生假体松动、断裂、脱位和假体周围骨折等严重远期并发症。髋关节功能按Harris评分:优11例,良18例,可3例,优良率90.6%。结论采用Ⅰ期人工全髋关节置换术能降低患者术后再手术率和术后髋关节疼痛发生率,是治疗有移位的老年股骨颈骨折的有效方法。更多还原

【Abstract】 Objective To explore the effect of one-stage total hip arthroplasty(THA) for treatment of displaced femoral neck fracture in elderly patients.Method 32 aged cases(32 hips) with displaced femoral neck fracture were treated by one-stage THA via improved Gibson approach.Results 32 cases were followed up for 12 to 36 months with an average of 23.4 months.Fat liquefaction of wound was found in 1 case and cured by debridment and change dressing.Deep vein thrombosis of unilateral low extremity occurred in 2 cases and restored by symptomatic treatment such as anticoagulation and affected limb elevation.No early complication of joint infection and pulmonary infection appeared,nor long-term complication as loosening,breaking,dislocation and adjacent fracture of prothesis.According to the Harris score,the hip joint function revealed excellence in 11 cases,good in 18 cases and fair in 3 cases,the fineness rate was 90.6%.Conclusion One-stage THA can reduce the rate of reoperation and postoperative hip pain;it is an effective treatment for displaced femoral neck fracture in aged patients.更多还原
