颅脑损伤急性期凝血指标和D-二聚体变化临床观察

【摘要】 目的探讨颅脑损伤急性期患者凝血指标和D-二聚变化特点以及与颅脑损伤严重程度、预后的关系。方法 (1)将264例急性颅脑外伤患者分为轻伤组(GCS>8分)和重伤组(GCS≤8分),测定病入院时的血浆凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(fibrinogen,Fbg)和D-二聚体(D-dimer),并与85例健康对照组比较。(2)治疗6个月后,根据GOS将病人分为两组(GOS1~3分组及GOS4~5分组),比较两组PT、APTT、Fbg和D-dimer值。结果 (1)各型颅脑损伤与对照组比较,PT、APTT、Fbg和D-dimer均有显著差异。(2)各型颅脑损伤中PT、APTT、Fbg和D-dimer均有异常,随着伤情加重PT、APTT和D-dimer均升高,但Fbg水平是降低。(3)治疗6个月后GOS1~3分组比GOS4~5分组PT、APTT、Fbg和D-dimer水平高,且有明显差异。结论急性颅脑损伤后存在凝血功能异常,且与颅脑损伤程度相关。可为判断急性颅脑损伤预后的提供依据积累了一定数据。

【Abstract】 Objective:To investigate characteristics of blood coagulation indexes and D-dimer in acute stage of head trauma and invent the prognosis of different levels.Methods:(1)264 cases of patients with head injury were divided into two groups:minor-injuried group(GCS,more than 8),severe-injuried group(GCS,less than 8),Levels of plasma PT,APTT,Fbg and D-dimer were detected and compared with that of 85 healthy cases;2)These patients were classified into two groups according to GOS after six-month treatment,GOS 4-5 and GOS 1-3.then to compare the levels of PT,APTT,Fbg and D-dimer between them.Results: PT,APTT,Fbg and D-diner are almost abnormal and have significance difference in acute craniocerebral injuries.The more the severity of head injury,the higher the levels of PT,APTT,Fbg and D-dimer are.Levels of PT,APTT,Fbg and D-dimer of GOS 1-3 are higher than that GOS 4-5 group.Conclusion: Head injury can result in coagulation disorder.This coagulopathy is related to the severity of head trauma and is a parameter to predict the outcome in head injury.
