平板数字旋转心室造影在复杂先天性心脏病中的应用

【摘要】 目的:与常规造影比较,探讨平板数字旋转造影在复杂先天性心脏病中的临床应用价值。方法:对比平板数字旋转造影组(122例)和常规造影组(610例)的复杂先天性心脏病患者的造影图像,详细记录各种主要畸形及解剖所见,按照心室形态、肺动脉形态(包括右室流出道,肺动脉瓣及肺动脉分支畸形)、室间隔缺损、房室瓣、主动脉及肺动脉的位置关系、主动脉骑跨度、冠状动脉开口及走行、PDA及侧枝血管八个方面评价显示满意度,并进行统计学分析。结果:在心室形态、室间隔缺损、房室瓣、主动脉及肺动脉的位置关系、主动脉骑跨度方面旋转造影和常规造影并无统计学差异,但是旋转造影在心室形态、肺动脉形态、主动及肺动脉位置上显示良好的病例百分比超过了常规造影。而在判断肺动脉形态、冠状动脉走行及开口方面上旋转造影要优于常规造影,统计学上具有显著性差异。与常规心室造影相比心室旋转造影更易于发现PDA及侧支血管。各种病例对比旋转造影较常规造影平均减少造影次数1~2次。结论:旋转造影较常规造影不但能提供更丰富的影像学信息,并减少了造影次数和对比剂用量,有较高的临床应用价值,可以提高诊断的准确性和介入治疗的安全性和疗效。

【Abstract】 Objective To evaluate the clinical application value of flat plate rotational digital angiography technique in complex congenital heart disease (CCHD) over general angiography. Methods CCHD patients were divided into two groups to undergo angiography examination, group1 (122 cases) examined with 2D rotational digital angiography and group2 (610 cases) with general angiography. Rates of well-observed anatomical morphology and abnormity were compared between two groups, including ventriculus, pulmonary artery (right ventriculu outflow tract (RVOT), pulmonic valve (PV), and pulmonary branches), ventricular septum, atrio-rentriculor valve, location of aorta and pulmonary artery, aortic overriding, coronary artery origin and course, patent ductus arteriosus (PDA), and main aorto-pulmonary collateral arteries (MAPCA). Results Though the rates of well-observed ventriculus, pulmonary artery, VSD, ventricular septum, atrio-rentriculor valve, location of aorta and pulmonary artery, aortic overriding in group1 are higher than those of group2, there is no statistically significant difference between these two groups. But the display of coronary artery origin and course, PDA and MAPCA in group1 were significantly better than those in group2. There were average 1-2 times decrease at angiography number in group1 than in group2. Conclusions Compared with common angiography, 2D rotational digital angiography not only decreases the number of angiography and the total volume of contrast media, bur also allows for accurate and comprehensive display of anatomical morphology and abnormity.
