鼻咽癌患者自体树突状细胞疫苗治疗对机体免疫功能的影响

【摘要】 目的比较鼻咽癌患者自体树突状细胞(DCs)疫苗治疗前后对患者机体免疫机能的影响,探讨热休克的鼻咽癌细胞抗原负载的鼻咽癌自体DCs疫苗激发机体免疫应答的能力与趋势。方法 20例鼻咽癌患者随机分为对照组及治疗组,对照组实施常规放疗,治疗组在常规放疗后给予1个疗程的DCs免疫治疗(4次皮内注射)。治疗过程中动态检测T细胞亚群、自然杀伤(NK)细胞、Th1类细胞因子(IL-2、IL-12、TNF-α、IFN-γ)水平,进行分析比较。结果 DCs治疗前与治疗后外周血CD3+、CD3+CD4+、CD4+/CD8+、NK细胞比率无明显变化(P>0.05);治疗后治疗组细胞因子IL-2、IL-12、TNF-α和IFN-γ的水平较治疗前明显升高(P<0.05)。结论鼻咽癌自体DCs疫苗免疫治疗可明显提升鼻咽癌患者Th1类细胞因子水平,激发Th1型免疫应答,增强机体抗瘤能力。

【Abstract】 Objective To comparing the effect of the treatment of autologus dendritic cells(DCs) vaccine on the immune status of nasopharyngeal carcinoma(NPC) patients.Methods Twenty NPC patients were included in this study.In the control group patients were treated with radioactive therapy and in the treatment group patients were treated with DCs vaccine after the radioactive therapy(hypodermic injection 4 times).The T cell subtype,the lever of cytokines in the serum were detected in the treatment group during the whole treatment process.Results Patients in the DC treated group after vaccination all have improvements on quality of life in different extend.The T cell subtype such as CD3+,CD3+CD4+,CD4+/CD8+,NK and the serous cytokines including IL-2,IL-12,TNF-α and IFN-γ,also have a significant rising(P<0.05).Conclusion Autologous DCs immunotherapy can improve the immune level of NPC patients.The level of cytokines in the serum of NPC patients after vaccination has a significant rising.This founding shows that DC immunotherapy induces the Th1 type response in NPC patients,and improves the anti-tumor ability.
