老年代谢综合征人群血尿酸水平及影响因素相关分析

【摘要】 目的探讨老年代谢综合征(MS)合并高尿酸血症的临床特征。方法将258例老年干部人群分为MS高尿酸血症组、MS正常血尿酸组和老年健康对照组,测定其腰围、体重指数(BMI)、血压(SBP,DBP)、血糖(FBG,PBG)、甘油三酯(TG)、胆固醇(CHO)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、空腹血清胰岛素(FINS)、血肌酐(Cr)、血尿酸(UA),游离脂肪酸(FFA),用最小稳态模型计算胰岛素抵抗指数(HOMA-IR)。结果与老年MS正常血尿酸组相比,老年MS高尿酸血症组的BMI、DBP、TG、FINS、HOMA-IR、FFA、Cr水平明显升高,而HDL-C水平降低。血尿酸与FINS、FFA、TG、HOMA-IR、BMI、FBG正相关,与HDL-C负相关。多元逐步回归分析显示:TG、HDL-C、HOMA-IR是老年MS合并高尿酸血症的独立危险因素,结论舒张压、血肌酐、甘油三酯、高密度脂蛋白、胰岛素抵抗与老年MS合并高尿酸血症密切相关,血尿酸水平的升高是代谢综合征的危险因素

【Abstract】 Objective To investigate the level of blood uric acid in old patients with metabolic syndrome and its influential factors.Methods 258 cases of old patients were divided into three groups(MS with hyperuricacidemia group,MS normal uric acid group,senium health control group).Waistline、BMI、blood pressure(SBP,DBP)、blood glucose(FPG,PBG)、triglyceride(TG)、cholestero(lTC)、high density lipoprotein(HDL-C)、low density lipoprotein(LDL-C)、fast serum insulin(FINS)、serum creatinine(Cr)、blood uric acid and serum free fatty acid(FFA)were determined in these patients.Insulin resistance index(HOMA-IR)was calculated by small homeostasis modal.Results The levels of BMI、DB P、TG、FINS、HOMA-IR、FFA、Cr in MS with hyperuricacidemia group were significantly higher than those in MS normal uric acid group.The level of HDL in MS with hyperuricacidemia group was significantly lower than in MS normal uric acid group.Blood uric acid levelw as positively correlated with FINS、FFA、TG、HOMA-IR、BMI、FBG.Blood uric acid level was negatively correlated with HDL-C.Multielement gradual regressive analysis disclosed that TG、HDL-C、HOMA-IR were the independent risk factors of MS with hyperuricacidemia crowd.Conclusion Diastolic blood pressure、serum creatinine、triglyceride、high density lipoprotein、insulin resistance were tightly associated with MS complicatedw ith hyperuricacidemia.The ascending level of blood uric acid was a risk factor of MS.
