2型糖尿病早期血管内皮功能变化的研究

【摘要】 目的探讨糖尿病肾病早期,血管内皮损伤标志物一氧化氮(NO)、内皮素(ET)、血管性血友病因子(vWF)的变化。方法收集2007年1—12月哈尔滨医科大学附属第二医院体检中心、内分泌科门诊及住院患者60例,测定正常人(A组)、尿微量白蛋白/肌酐(A/C)正常的糖尿病患者(B组)、A/C升高(C组)的早期糖尿病肾病患者血中的ET、NO、vWF。结果 ET随糖尿病肾病的发生逐渐升高[(30.69±7.39)ng/L对(49.48±14.37)ng/L对(73.07±20.34)ng/L,P<0.05],且与糖尿病病程呈正相关(P<0.05);vWF在C组显著升高[(19.90±6.97)μg/L对(28.13±9.27)μg/L];血清NO在B组明显升高[(54.89±9.41)μmol/L对(77.45±10.58)μmol/L,P<0.05]。结论早在临床诊断肾病之前,肾脏内皮细胞的损伤可能已经开始发生,而肾脏内皮细胞的损伤将进一步加剧糖尿病肾病的进展。

【Abstract】 Objective To observe the changes of NO,ET-1,and vWF,what is the markers for vascularendothelial injury,in the early diabetic nephropathy(DN).Methods There were three groups,group A:healthy control group;group B:diabetes with normal albuminuria,group C:diabetes with trace albuminuria.Blood samples were collected in the morning,and the value of serum NO,ET-1,and vWF were determined.Results The value of NO was significantly increased in group C [group A vs group C,(54.89±9.41)μmol/L vs(77.45±10.58)μmol/L,P<0.05];ET-1 increased gradually by diabetic nephropathy happens[(30.69±7.39)ng/L,(49.48±14.37)ng/L,and(73.07±20.34)ng/L in group A,B and C,P<0.05];vWF increased significantly in group C[group B vs group C,(19.90±6.97)μg/L vs (28.13±9.27)μg/L,P<0.05].Conclusion The injury of renal vascularendothelial may happen before the clinical diagnosis of diabetic nephropathy,and the injury would speeds up the development the diabetic nephropathy.
