【摘要】 目的 以维质子磁共振波谱分析(magnetic resonance spectroscopy,MRS)的方法定量分析前列腺中央腺体偶发癌(IDPC)与良性前列腺增生(BPH)的代谢特征。方法 对103例行尿道前列腺切除术(transurethral resection of the prostate,TURP)的前列腺中央腺体偶发癌和良性前列腺增生患者做回顾性分析,比较两者(胆碱+肌酸)/枸橼酸盐[(Cho+Cre)/Cit,CC/C]值,然后再根据病理结果将良性前列腺增生的患者分为3组:以腺体增生为主型4例,以间质增生为主型30例、混合型增生62例,比较中央腺体偶发癌组与各组增生之间CC/C值。结果(1)前列腺中央腺体偶发癌组的CC/C比值(1.04±0.28)与良性前列腺增生组(1.09±0.59)差异无统计学意义(t=-0.217,P=0.829)。(2)中央腺体偶发癌组CC/C比值小于以间质增生为主型(1.31±0.94),大于以腺体增生为主型(1.00±0.03)和混合型增生组(0.99±0.31),组间比较差异无统计学意义(χ~2=2.595,P=0.458)。结论 MRS不能鉴别前列腺中央腺体偶发癌与良性前列腺增生。更多还原

【Abstract】 Objective To quantify and compare the metabolic characteristics of incidentally detected prostate carcinoma(IDPC)in the central gland of prostate and benign prostate hyperplasia(BPH) by using MR spectroscopy.Methods The clinical and imaging data of 103 patients with BPH and IDPC who underwent TURP were analyzed retrospectively,and the Cho+Cre/Cit(CC/C) values of the two groups were compared.The patients with BPH were divided into three groups:4 cases with glandular-BPH,30 cases with stromal-BPH,and 62 cases with mixed-BPH,the CC/C value of each group was compared with IDPC.Results(1)There was no significant difference between the groups of IDPC(1.04±0.28) and BPH(1.09±0.59)(t=-0.217,P=0.829).(2)The mean CC/C value of IDPC was lower than that of stromal-BPH(1.31±0.94),higher than that of glandular-BPH(1.00±0.03)and mixed-BPH(0.99±0.31),and no significant difference(χ~2=2.595,P=0.458) could be detected between groups.Conclusion MRS do not have benefits in the differential diagnosis of IDPC and BPH.更多还原
