腰椎休门氏病的MRI诊断及鉴别诊断
MRI Diagnosis and Differential Diagnosis of Lumbar Scheuermann’s Disease
【摘要】 目的 探讨腰椎休门氏病的MRI表现,提高其诊断及鉴别诊断水平。方法 结合文献对27例腰椎休门氏病的MRI表现进行回顾性分析。结果 腰椎休门氏病MRI表现为病变腰椎椎体上、下终板不规则和Schmorl结节形成,在T2WI表现为低信号或高低混杂信号,T1WI为低信号。无明显椎间盘突出及膨出征象,无脊柱畸形。病变主要发位于T12以下相邻椎体缘。发生在T11~12椎间隙3例,T12~L1椎间隙9例,L1~2椎间隙14例L2~3椎间隙13例,L3~4椎间隙22例,L4~5椎间隙12例L5~S1椎间隙15例。病变累及1个椎体缘1例,2个椎体缘3例,3个椎体缘10例,4个椎体缘6例,5个椎体缘5例,6个椎体缘2例。结论 正确认识腰椎休门氏病的MRI表现,有利于正确诊断,减少误诊。
【Abstract】 Objective To study MR imaging features of lumbar Scheuermann’s disease so as to improve its diagnosis and differential diagnosis.Methods MR features of lumbar Scheuermann’s disease in 27 patients were retrospectively analyzed with literature review.Results MRI demonstrated that the shape of the vertebral endplate was irregular with Schmorl’s nodes,T2 WI showed hyperintensity and heterogeneous intensity.T1WI showed hypointensity.Intervertebral discs were normal.The lesions mainly located at vertebral limbus inferior thoracic vertebrae 12.The lesion located at T11~12 intervertebral space in 3 cases,T12~L1 intervertebral space in 9 cases,L1~2 intervertebral space in 14 cases,L2~3 intervertebral space in 13 cases,L3~4 intervertebral space in 22 cases,L4~5 intervertebral space in 12 cases and L5~S1 intervertebral space in 15 cases.The lesion involving 1 vertebral border in 1 case,2vertebral border in 3 cases,3 vertebral border in 10 cases,4 vertebral border in 6 cases,5 vertebral border in 5 cases and 6 vertebral border in 2 cases were seen.Conclusion To correctly judge the MRI manifestations of lumbar Scheuermann’s disease can improve its diagnosis and differential diagnosis.
