64排VCT扫描结合多种后处理技术在胃癌的诊断价值

【摘要】 目的 评价64排VCT扫描结合多种后处理技术在胃癌诊断中的临床应用价值。方法 回顾性分析经病理证实的41例胃癌,均行64排VCT胃充气扫描及四期增强扫描,将所得容积数据传至工作站,行表面遮盖显示(SSD),透明胃(Raysum),仿真内窥镜(CTVE),多平面重建(MPR),曲面重建(CPR),动脉血管重建(CTA),所得结果与胃镜和/或手术病理对比。结果 41例胃癌,轴位图像结合多种后处理技术,对胃癌诊断的准确率为97.56%(40/41),对38例进展期胃癌大体分型准确率为100%(38/38)。轴位图像结合CPR及MPR能客观地反映出胃壁病变的部位、形态及大小,确定病变的侵犯程度、胃周脏器的侵犯程度、肿大淋巴结的检出及远处脏器转移情况。SSD、Raysum及CTVE能显示本病的部位、立体形态、大小及范围。CTA均能重建出胃癌的供血动脉。结论 64排VCT扫描与多种后处理技术相结合,能对胃癌进行综合评估,具有较高的临床应用价值。

【Abstract】 Objective To evaluate 64-VCT scanning and multiple reformations in the clinical diagnosis of gastric carcinoma.Methods 41 patients with gastric carcinoma proved pathologically were analyzed retrospectively.All patients underwent plain CT scan at gaster with contrast gas and four phases contrast-enhanced scan.The voulumtric data were transferred to the computer workstation to reformat the images as SSD,Raysum,CTVE,MPR,CPR,CTA.The results were compared with the findings of pathology.Results In 41 cases,the diagnostic accurate rate with axial images in combination with multiple reformations was 97.56%(40/41),and the accurate rate for advanced gastric carcinoma in 38/41 cases was 100%.Axial images in combination with MPR,CPR could demonstrate the location,size,extension of the lesion,the invasion of organs around the lesions,metastatic lymph nodes and distant metastasis.SSD,Raysum,CTVE could demonstrate the three-dimensional images of the lesions,size,and extension of the lesions.CTA could show all artery vessel of the gastric carcinoma.Conclusion 64-VCT scanning and its multiple reformations can evaluate the gastric carcinoma synthetically,and is of great value in diagnosis of gastric carcinoma.
