原发性高血压患者尿白蛋白/尿肌酐比与股动脉僵硬度的相关性

【摘要】 目的研究尿微量白蛋白与股动脉僵硬度之间的相关性,探讨高血压早期靶器官损害的敏感指标。方法将128例研究对象分为正常对照组(n=35)、高血压非蛋白尿组(A组,n=50)和高血压蛋白尿组(B组,n=43),测定体质量指数(BMI)、空腹血糖(FPG)、甘油三酯(TG)、总胆固醇(TC)、血清肌酐(Cr)、计算肾小球滤过率(GFR)、高密度脂蛋白胆固醇(HDL)、低密度脂蛋白胆固醇(LDL)、尿素氮(BUN)、收缩压(SBP)、舒张压(DBP)、脉压(PP)等指标;采用快速免疫比浊法测定尿白蛋白/尿肌酐比(ACR);应用超声心动图测量股动脉(CFA)内膜中层厚度(IMT)、收缩期峰值速度(PSV)、舒张末期峰值速度(EDV)、收缩期内径(Ds)、舒张期内径(Dd)并计算股动脉僵硬度、紧张度及扩张度;应用动脉弹性检测仪测定颈-股动脉波传播速度(cf-PWV)。结果与正常对照组相比,A组与B组的ACR、cf-PWV、股动脉僵硬度均升高(P<0.05);B组股动脉的紧张度及扩张度降低(P<0.01);ACR与cf-PWV、僵硬度呈正相关(P<0.01),与紧张度及扩张度呈负相关(P<0.05)。结论 ACR、cf-PWV及股动脉僵硬度可作为评价原发性高血压病患者早期靶器官损害、心血管疾病危险性的无创性敏感指标。

【Abstract】 Objective To explore the association of microalbuminuria( MA) with femoral arterial stiffness and sensitive indicators of early target organ damage in hypertensive patients. Methods 128 individuals were divided into three groups: the healthy controls ( n = 35) ,patients with normal microalbuminuria ( group A,n = 50) ,and microalbuminuria ( group B,n = 43) . Body mass index ( BM I) ,fasting blood glucose ( FPG) ,triglycerides ( TG) ,total cholesterol ( TC) ,serum creatinine( Cr) ,glomerular filtration rate ( GFR) ,high density lipoprotein cholesterol ( HDL) ,low density lipoprotein cholesterol( LDL) ,blood urea nitrogen( BUN ) ,systolic pressure( SBP) ,diastolic pressure( DBP) and pulse pressure( PP) were measured in all subjects. Urinary albumin-to-creatinine ratios( ACR) were measured with immunoturbidimetry ; femoral arterial intima-media thickness ( IM T) ,peak systolic velocity ( PSV) ,end diastolic velocity ( EDV) ,systolic diameter ( Ds) and diastolic diameter ( Dd) were measured with ultrasonography; stiffness,tensity and distensibility of the femoral artery were calculated; pulse wave velocity( cf-PWV) were measured with an arterial elasticity meter. Results Compared with the controls,ACR,cf-PWV and stiffness in group A and group B were significantly increased ( P < 0. 05) ; tensity and distensibility in group B were significantly decreased ( P < 0. 01) ; ACR was positively related with PWV and stiffness ( P < 0. 01) and negatively related with femoral tensity and distensibility ( P <0.05) . Conclusion ACR,cf-PWV and femoral arterial stiffness are non-invasive and sensitive indicators,which could predict early target organ damage and cardiovascular risk in hypertension.
