【摘要】 目的探讨不同孕周的50g葡萄糖负荷试验(50gGCT)对筛查妊娠期糖尿病(GDM)的临床意义以及与孕妇年龄、体重及孕周的关系。方法回顾性分析2006年1月～2009年3月在我院围产保健科接受糖筛查的孕妇4809例,按孕龄分为3组:A组<24周,B组24～28周,C组>28周,对GCT阳性的全部进行75g糖耐量(OGTT)试验,血糖结果分为正常组、GIGT、GDM3组,根据孕妇年龄和体重因素,进行分组对照研究。结果 GCT与OGTT异常检出率在A、B、C 3组间对照差异无统计学意义(P>0.05)。GCT与OGTT异常检出率在不同年龄、不同体重组间差异有统计学意义(P<0.05),<25岁无高危因素的孕妇GDM的发生率显著低于年龄≥25岁的孕妇。孕妇的平均体重指数(BMI)≥25.1时GCT异常率和GDM发生率均显著高于BMI<25.1者。结论孕期均适宜50g葡萄糖筛查试验而非仅限于24～28周;孕妇的年龄和体重与GDM的发生密切相关,对肥胖或高龄孕妇应尽早行GCT筛查。更多还原

【Abstract】 Objective To study the clinic value of the 50 gram glucose challenge test(GCT) to screen the gestational diabetes mellitua(GDM) in different pregnant weeks,and the relationships of maternal age,body weight and pregnant weeks to GDM.Methods The clinical data of 4809 pregnant women who underwent the GCT from Januuary 2006 to March 2009 in Perinatal Healthcare department of our hospital were collected,All women were divided into three groups :(A group)less than 24 weeks of gestation group,(B group) 24～28 weeks of gestation group and(C group) more than 28 weeks of gestation group.75 gram glucose tolerance test(OGTT) were performed in all the women who were positive in GCT and they were divided into three groups:normal group,Glucose intolerance(GIGT) group;GDM group.A control study were made based on pregnant women′s age and weight.Rerults ①We compared A,B and C group,there were no significant difference in the abnormal rate of GCT and GDM(P<0.05).②There were significant difference of the abnormal rate of GCT and GDM when compared by pregnant women′s age and weight(P<0.05).The incidence rate of GDM of pregnant women younger than 25 years old without high risk factors was significantly lower than women older than 25 years.The incidence rate of GCT and GDM of pregnant women whose BMI were on less than 25.1 were significantly higher than that of BMI lower than 25.1.Conclusions ①It is suitable to screen GDM during gestation not only for 24 to 28 weeks.②Age and weight of pregnant women are the important risk factors for the GDM.It is necessary to screen GDM as soon as possible for pregnant women with obesity or older age.更多还原
