【摘要】 目的探讨呼吸道病毒感染在过敏性紫癜发病机制中的作用。方法采用酶联免疫吸附法观察40例过敏性紫癜血浆中呼吸道合胞病毒、腺病毒、流感病毒、副流感病毒IgM抗体,直接免疫荧光法观察该40例血液中单个核细胞及肾组织中呼吸道合胞病毒、腺病毒、流感病毒A、流感病毒B、副流感病毒1、2、3抗原,分别比较A组(外周血病毒抗原检测阳性)与B组(外周血病毒抗原检测阴性),A1组(外周血病毒抗原检测阳性,肾组织中病毒抗原检测阳性)与A2组(外周血病毒抗原检测阳性,肾组织中病毒抗原检测阴性)的临床及病理特征。结果①过敏性紫癜组副流感病毒的检出率(12/40,30%)高于呼吸道感染组(1/42,2.4%),差异有统计学意义(χ2=11.715,P<0.05),呼吸道合胞病毒、腺病毒、流感病毒的检出率两组之间差异无统计学意义(P>0.05)。②肾组织,呼吸道病毒抗原13例阳性(13/40,32.5%),其中副流感病毒12例(12/13)(副流感病毒Ⅱ10例,副流感病毒Ⅲ2例),1例为呼吸道合胞病毒。抗原在肾小管的荧光强度及密度明显高于肾小球。结论呼吸道病毒,特别是副流感病毒感染可能与过敏性紫癜发病有关。更多还原

【Abstract】 Objective To explore the role of respiratory tract virus infection in the pathogenesis of HSP.Methods 40 cases of Henoch-Schonlein Purpura and in 42 matched-controls,we tested for the presence of parainfluenza virus,influenza virus,respiratory syncytial virus and adenovirus in plasma,peripheral blood mononuclear cells,and renal tissue.Results In plasma and peripheral blood mononuclear cells,there were no significant differences in the presence of viruses between the Henoch-Schonlein Purpura and control groups.However,the rate of parainfluenza virus infection was significantly higher in the plasma and peripheral blood mononuclear cells of Henoch-Schonlein Purpura patients than control patients(12/40 vs.1/42;P=0.001).A total of 13 Henoch-Schonlein Purpura patients were positive for viruses in renal tissue(10 parainfluenza virus-Ⅱ,2 parainfluenza virus-Ⅲ,and 1 respiratory syncytial virus).In these 13 children,the plasma IgM and peripheral blood mononuclear cells antigens were also positive.Viral antigens were more abundant in renal tubules than the glomerulus.There were no significant differences between the groups in histopathology.The small number of patients with virus positive limited the statistical power of the analyses.Conclusion Respiratory tract viruses,especially parainfluenza virus,may play a role in the pathogenesis of Henoch-Schonlein Purpura.更多还原
