六分钟步行试验评判慢性阻塞性肺疾病肺功能影响因素分析
Influencing Factors to Six Minute Walk Test for Assessing Pulmonary Function of Chronic Obstructive Pulmonary Disease
【摘要】 目的探讨肺功能分级、年龄、性别、体质指数是否影响6 min步行试验评价慢性阻塞性肺疾病(COPD)患者肺功能变化。方法选择2008年6月—2009年7月于我院呼吸内科就诊的稳定期COPD患者94例,进行6 min步行试验并测定肺通气功能。结果 94例COPD患者肺功能指标FVC、FEV1、FEV1%、FEV1/FVC、PEF、IC实测值与6 min步行距离均成正相关(P<0.05);根据肺功能、年龄、性别及体质指数对94例患者进行分组,中重度肺功能(Ⅱ、Ⅲ级)、低龄患者以及不同性别组、不同体质指数组大部分肺功能指标与6min步行距离之间有相关性(P<0.05);极重度肺功能(Ⅳ级)、高龄患者的大部分肺功能指标与6 min步行距离之间无相关性(P>0.05)。结论 6 min步行试验适用于评估中重度肺功能、年龄<65岁的COPD患者肺功能变化,能否用于评估极重度肺功能、高龄患者的肺功能变化值得商榷。
【Abstract】 Objective To investigate whether lung function grading,age,gender and body mass index(BMI) influence six minute walk test(6MWT) for assessing pulmonary function of the patients with chronic obstructive pulmonary disease(COPD).Methods Pulmonary function and 6MWT were detected in 94 patients with COPD in stable stage in our hospital from June 2008 to July 2009.Results The indexes of pulmonary function FVC,FEV1,FEV1%,FEV1/FVC,PEF,and IC in 94 COPD patients were positively correlated with the really detected values of 6 minute walk distance(P<0.05).The 94 patients were grouped according to their pulmonary function,age,gender,and BMI.And then the majority of pulmonary function were correlated with the corresponding values of 6 minute walk distance in patients of the group of middle-serious damaged pulmonary function(graded Ⅱor Ⅲ),lower aged group,different gender groups,and different BMI group(P<0.05);while there was no correlation between the majority of pulmonary function and the corresponding values of 6 minute walk distance in patients of the group of critically serious damage to the pulmonary function(graded Ⅳ) and advanced aged group(P>0.05).Conclusion The 6MWT is suitable for assessment of pulmonary function of COPD patients with graded Ⅱ and Ⅲ pulmonary function or aged younger than 65.However it is doubtful for that of COPD patients with graded Ⅳ pulmonary function or aged older than 65.
