舒芬太尼复合罗哌卡因硬膜外给药在阑尾手术中的应用

【摘要】 目的探讨舒芬太尼复合0.75%罗哌卡因硬膜外给药在阑尾手术中的应用意义。方法急诊行阑尾手术病人60例随机分为两组,治疗组(n=30,舒芬太尼15μg加0.75%罗哌卡因15ml),对照组(n=30,0.75%罗哌卡因15ml),记录两组的感觉阻滞起效时间、感觉最高阻滞平面、达最高平面时间、镇痛持续时间,并对术中内脏牵拉反应及镇静分线(Ramsay评分)进行评估。结果与对照组比较,治疗组的感觉阻滞起效时间及达最高平面的时间缩短,感觉最高阻滞平面升高,镇痛持续时间延长,差异均有统计学意义(t分别=7.34、7.69、6.10,P均<0.05),内脏牵拉反应减少,Ramsay评分在Ⅱ~Ⅲ级人数较多,差异均有统计学意义(χ2分别=5.33、7.18,P均<0.05)。结论舒芬太尼复合0.75%罗哌卡因硬膜外麻醉在阑尾手术中的应用能取得更加满意的麻醉效果。

【Abstract】 Objective To investigate the application meaning of epidural medicine preparations with sufentanil and 0.75% ropivacaine at appendectomy. Methods Sixty patients undergoing appendectomy were randomly divided into two groups: treatment group (n=30, 0.75% ropivacaine 15ml including sufentanil 15μg); contrast group(n=30, 0.75% ropivacaine 15 ml). The time of effect-acting, the highest plane of feeling block, the time to the highest plane, and the analgesia time were recorded, and the calmness of the patients and internal force reaction were assessed. Results Compared to the contrast group, the time of effect-acting and the time to the highest plane were shorter, the highest plane of feeling block was higher, the analgesia time was longer in the treatment group; there were statistically significant differences (t=7.34,7.69,6.10, P<0.05). Internal force reaction obviously decreased, the number of patients in Ramsay score II and III increased; there were statistically significant differences (χ2 =5.33,7.18, P <0.05). Conclusions Application of epidural medicine preparations with sufentanil and 0.75% ropivacaine for appendectomy has more satisfying anesthetic effect.
