326株尿路感染病原菌分布及耐药情况分析
Distribution of pathogenic bacteria in urinary and their drug resistance
【摘要】 目的:分析我院尿路感染的菌群分布及耐药性情况,为临床合理使用抗生素提供可靠依据。方法:使用法国生物-梅里埃公司VITEK-2全自动细菌鉴定系统对获得的326株主要致病菌进行细菌鉴定及药敏试验。结果:在分离的病原菌中,G-杆菌242株占74.23%,以大肠埃希菌最多,占56.13%;G+球菌77株占23.62%,以肠球菌为主占11.35%;真菌7株占2.15%。产ESBLs的大肠埃希菌检出率为44.26%(81株)、肺炎克雷伯菌检出率为36.36%(8株)。药敏显示革兰氏阴性杆菌对碳青霉稀类药物(亚胺培南)耐药率最低,对青霉素类耐药率最高。革兰氏阳性球菌对万古霉素耐药率最低,耐甲氧西林葡萄球菌占很大比例,达到76.8%。结论:大肠埃希菌仍是细菌性尿路感染的主要病原菌,而产ESBLs已占很大比例。建议临床医生重视细菌培养,在药敏提示下使用抗生素,以减少耐药菌株的产生。
【Abstract】 Objective To analyze the bacteria distribution and drug resistance Characteristics in urinary tract infection for providing a basis to select and use antibiosis medicine in clinic.Methods Using the France BIOMERIEUX company VITEK-2 bacterial indentificationinstrument to identify 326 strains of clinical pathogen bacteria and conduct sensitivity test.Results In all the bacteria,Gram-negative bacteria were74.23%,the primary was.Escherichia coli(56.13%),Gram-postive bacteria were 23.62%,the primary was Enterococcus(11.35%) and 7 strains of fungus(2.15%).The prevalence of ESBLs-producing E.coli and Klebsiella SPP was 44.26% (81 strains) and 36.36% (8 strains).The resistance rates of Gram-negative bacteria was the lowest to imipenem. The resistance rates of Gram-postive was the lowest to Vancomycin.The prevalence of MRSA/MRSE was 76.8% Conclusion The Escherichia coli is the primary pathogen bacteria in urinary tract infection.Clinicians should pay attention to bacterial culture and use drugs more reasonable,so as to reduce the multi-drug resistance bacteria.
