内镜下碘染色技术在早期食管癌及癌前病变的应用价值

【摘要】 目的:探讨内镜下碘染色技术在诊断早期食管癌及癌前病变的临床应用价值。方法:对门诊接受内镜检查时发现食管黏膜有可疑病变者进行碘染色检查,记录碘染不良病灶的部位、大小、形态和边界等,并行多点活检,然后将食管鳞状上皮的着色情况与其对应的病理检查结果进行对比分析。结果:478例染色前发现食管可疑病变532个,染色后405例出现不染及淡染区586个,约1/3的患者可见2个以上不染或淡染区。中度不典型增生以上者168例,占28.7%。结论:内镜下碘染色技术可提高早期食管癌及癌前病变的检诊率。

【Abstract】 Objective:To investigate the clinical value of esophageal mucosal iodine stain during esophagoscopy for patients with early esophageal carcinoma or precancerous lesions,through analyzing the correlation between endoscopic findings and pathological results of biopsy on the suspicious spots. Methods: Patients examined by iodine stain during esophagoscopy,the position,size,shape and boundary of all visible unstained lesions were recorded and multiple biopsies were taken on the unstained spots.Results: Before iodine stain,532 lesions had been discovered in 478 patients. However,586 abnormal lesions stained in 405 patients were detected after iodine stain.More than 1/3 of patients were found to have more than 2 abnormally stained lesions.28.7% of them(168 cases) had moderate or severe dysplasia or early esophageal cancer.Conclusion: Esophageal mucosal iodine stain showed highly sensitivity and specificity for detecting these precancerous and early squamous cancer lesions,it may be helpful for diagnosis of early esophageal carcinoma and precancerous lesions.
