性激素水平与颅内动脉瘤发生机制的相关性研究

【摘要】 目的通过测量并分析男、女颅内动脉瘤患者与对照组(非动脉瘤患者)的性激素水平,探讨性激素水平与颅内动脉瘤发生的关系。方法对1992年1月至2008年12月脑血管造影诊断的颅内动脉瘤1768例患者的年龄及性别,分析其发病年龄高峰及性别差异。并对2009年1月~12月因蛛网膜下腔出血来我院患者行脑血管造影检查,确诊动脉瘤的成年患者为研究组,男76例、女97例共173例,检测其空腹静脉血雌二醇(E2)、睾酮(T)、促卵泡成熟素(FSH)、促黄体激素(LH)、孕酮(P)、血清泌乳素(PRL)浓度,对照组为同期住院行DSA检查排除动脉瘤的成年患者177例,并检测其空腹静脉血上述激素浓度。分析研究组与对照组性激素间的关系。结果 1768例患者年龄分布在10~88岁之间,高峰在50~59岁年龄段,占总数的32.41%,40~69岁年龄段占60.46%,40岁以前动脉瘤患者男性多于女性,40岁以后女性多于男性。单发动脉瘤男:女=1:1.28;多发动脉瘤患者约占总数的12.78%,男:女=1:3.71。雌二醇水平在女性研究组和对照组均值分别为51.9±62.37pmol/L和107.58±60.45pmol/L,围绝经期32.80±28.18pmol/L和111.00±60.37pmol/L,绝经后期20.10±19.17pmol/L和41.40±23.12pmol/L,研究组和对照组雌二醇水平差异有统计学意义(p<0.05);月经周期正常98.33±82.81pmol/L和100.20±33.20pmol/L,研究组和对照组雌二醇水平差异无统计学意义(p>0.05)。睾酮水平在男性研究组和对照组均值分别为2.669±1.65nmol/L和4.43±1.93nmol/L,50~59岁2.77±1.97nmol/L和4.78±2.11nmol/L,60岁以上2.09±1.47nmol/L和3.94±2.21nmol/L,研究组和对照组的睾酮水平差异有统计学意义(p<0.05);20~49岁3.52±1.93nmol/L和4.33±1.61nmol/L,研究组和对照组睾酮水平差异无统计学意义(p>0.05)。结论动脉瘤的发病年龄高峰在50~59岁,雌二醇及睾酮随年龄增长下降。随着年龄的增长性激素水平的明显降低并低于正常性激素水平是动脉瘤发生、发展的一个重要因素之一。

【Abstract】 Objective By measuring sex hormone levels in different ages of male and female patients with intracranial aneurysms and the control group(non-aneurysm patients)to explore relationship between the levels of sex hormone and the formation of intracranial aneurysm.(CA)Methods Collect continuity 1768 cases of intracranial aneurysm patients ages and genders between Jan.1992,Dec.2008,which has been diagnosed with intracranial aneurysms,then analysis of their peak age of onset and sex differences.Analyse brain angiography images of patients contracted with cerebral hemorrhage between Jan.2009 to Dec.2009.There are 173 CA patients and 177 non-aneurysm patients as the control group.The control group are patients in which no intracranial aneurysm seen in digital subtraction arteriography(DSA).Detection of the estradiol(E2),testosterone(T),follicle-stimulating principle(FSH),lutein stimulating hormone(LH),progestone(P),Serum prolactin(PRL)concentration in all patients with their fasting blood.To study the sex hormones relationship between CA group and the control group.Results The age distribution of 1768 cases was fromthe 10 years of age to 88 years old.The peak age was 50~59-year-old group(32.41%).There were 60.46 percentages in the 40~69 age group.Before the age of 40 aneurysms in patients with more men than women,but more women than men after the age of 40.Female single aneurysms were 1.28 times the males.The multiple aneurysms were found in 12.78%,and the female was above triple of male in E2 level of CA group in total,menopausd women and postmenopausal women were significantly lower than that of control group(p<0.05);the difference of E2 in premenapausal women had no statistically significant(p>0.05).Conclusion The peak age of onset of aneurysms is in 50 ~ 59-year-old group.Both of estradiol(E2)and testosterone(T)decreased greatly with aging.It is an important facter in formation and development of intracranial aneurysm that sex hormone decreases greatly with aging and especially when it is lower than normal level.
